乳腺磁共振成像诊断乳管内乳头状瘤的临床价值

Value of magnetic resonance imaging in diagnosis of intraductal papilloma

摘要 目的:探讨乳腺磁共振成像(MRI)诊断乳管内乳头状瘤的临床价值。方法:选择176例有术前乳腺MRI资料,行手术治疗并病理证实的女性患者,其中乳管内乳头状瘤37例、非乳管内乳头状瘤的良性病变52例和乳腺癌87例。比较3组患者临床、病理及MRI资料,后者包括时间-信号强度曲线、影像学表现、肿瘤表观弥散系数(ADC)值。结果:37例乳管内乳头状瘤患者病灶37处,52例非乳头状瘤的良性病患者病灶56处,87例乳腺癌患者病灶92处。乳管内乳头状瘤组患者与其他两组患者比较,在发病年龄方面无统计学差异(F=2.470,P=0.088),但在肿瘤大小、肿瘤位置、时间-信号强度曲线、MRI下表现、ADC值等方面均有统计学差异(均P<0.05)。结论:乳管内乳头状瘤具有独特MRI的影像学特征,故辅以ADC值,MRI可以成为临床诊断乳管内乳头状瘤的重要手段。 Objective: To assess the clinical value of magnetic resonance imaging (MRI) in diagnosis of intraductal papilloma. Methods: One hundred and seventy-six female patients having preoperative MR] data, undergoing surgical treatment and with pathological confirmation were selected. Of the patients, there were 37 cases ofintraductal papilloma, 52 cases of non-papilloma benign lesions, and 87 cases of breast cancer. Comparison among the three groups of patients was performed in regard to clinical, pathological and MRI data, and the latter included time-intensity curve, imaging findings, and apparent diffusion coefficient (ADC). Results: Thirty-seven lesions were found in the 37 intraductal papilloma, 56 lesions were found in the 52 patients with non-papilloma benign diseases, and 92 lesions were found in the 87 breast cancer patients,respectively. Compared with patients in other two groups, patients with intraductal papilloma showed no difference in age of onset (F=2.470, P=0.088), hut was statistically different in lesion size, location, time- intensity curve, imaging manifestations, and ADC value (all P〈0.0S). Conclusion: Intraductal papilloma has distinct MR] characteristics, so, with support of ADC value, MR] can be used as an important approach for clinical diagnosis ofintraductal papilloma.
