摘要
目的:探讨氟比洛芬酯复合右美托咪定全身麻醉后,对抑制苏醒期躁动的有效性.方法:择期手术患者80例,随机分为2组(n=40).F组:术前10min氟比洛芬酯1mg/kg静注;FD组:术前10min氟比洛芬酯1mg/kg+术毕前5min给予右美托咪定0.5μg/kg缓慢静注.记录患者基础(T0)、拔管前1min(T1)、拔管后1min(T2)、拔管后5 min(T3)、拔管后10min(T4)的HR、MAP值并评估患者T3、T4的Ramsay镇静(RSS)评分和VAS疼痛评分,记录患者苏醒、拔管时间.结果:T1 ~T4时间点,FD组HR、MAP升高幅度均显著小于F组(均P<0.05).RSS评分:T3、T4时间点,F组与FD组相比评分显著降低(P<0.05).VAS评分:F组与FD组相比评分显著升高(P<0.05).唤醒、拔管时间方面2组相比差异无统计学意义(均P>0.05).结论:术前10min给予氟比洛芬酯1mg/kg+术毕前5min给予右美托咪定0.5μ g/kg,可有效降低术后躁动且不影响苏醒、拔管时间.
Objective:To investigate the effectivness of flurbiprofen combined with dexmedetomidine to prevent restlessness during the stage of analepsia.Methods:Eighty patients received elective general anesthesia were randomly divided into two groups (n=40).Group F:flurbiprofen 1mg/kg intravenously 10min before surgery; group FD:flurbiprofen 1mg/kg intravenously 10min before surgery combined with dexmedetomidine 0.5μg/kg intravenously 5min before the end of surgery.The heart rate(HR)and mean artery pressure(MAP)were recorded before anesthesia(T0),before extubation 1min(T1),after extubation 1min(T2),5min(T3),10min(T4).The time of analepsia and extubation were recorded.The Ramesay sedation (RSS),visual analogue scale (VAS)scores were also recorded at the time points T3 and T4.Results:The group FD were lower than group F in the changes of HR,MAP at the time points T1~T4,the difference was statistically significant(P<0.05).The group F were lower than group FD in the changes of RSS at the time points T3 and T4,the difference was statistically significant (P<0.05).The group F were higher than group FD in the changes of VAS at the time points T3 and T4,the difference was statistically significant (P<0.05).Conclusion:Using flurbiprofen 1mg/kg intravenously 10min before surgery combined with dexmedetomidine 0.5μg/kg intravenously 5min before the end of surgery can effectively prevent of restlessness in the stage of analepsia and does not affect the time of analepsia and extubation.
