脾多肽对油船烧伤患者免疫调节作用的临床观察 被引量:11

摘要 目的探讨脾多肽对油船烧伤患者免疫调节作用的影响。方法入选的42例油船烧伤患者,经患者知情同意分成2组。治疗组21例伤后第3天起接受脾多肽注射液治疗,6ml脾多肽加入0.9%生理盐水250ml,静脉滴注,疗程14d;对照组2l例不给予脾多肽注射液治疗,其余常规治疗2组相同。检测2组治疗前与治疗后第7、14、21天外周血T淋巴细胞(CD3)总数及亚群(CD4、CD8、CD4/CD8)数,治疗前、治疗后第7天血清免疫球蛋白(IgA、IgG、IgM)水平,记录治疗后第7、14、21天血液细菌培养及创面细菌培养阳性人数和病程中并发相关系统感染人数。结果治疗组应用脾多肽注射液后CD4升高,CD8减少,CD4/CD8升高,治疗后第7天IgA、IgG水平优于对照组,差异有统计学意义(P〈0.05),感染情况少于对照组。结论脾多肽注射液作为免疫调节剂,可有效改善油船烧伤患者免疫力。 Objective To investigate the effect of lienal polypeptide (LP) on the immune function of burn patients aboard ships. Methods Forty-two burn patients were randomly divided into 2 groups: the treatment group (21 eases) and the control group (21 cases). On the 3rd day after injury, the patients in the treatment group were treated intravenously with 6 ml LP injection, which was diluted with 0.9% physiological saline, while the patients in the control group received the same routine treatment, but without intravenous LP solution, all with a treatment course of 14 days. At day 2, 7, 14 and 21 after injury, total T lymphocytes in peripheral blood (CD3) and T lymphocyte subgroup (CD4, CD8, CD4/CD8 ) were detected in both the treatment group and the control group. Levels of immune globulin (IgA, IgG, IgM) were also detected 10 days before and after therapy. The No. of positive cases with blood bacteria culture and wound surface bacteria culture and the No. of associated system infection were recorded on the 7th, 14th and 21st days after injury. Results Following treatment with LP, the level of CIM in the treatment group increased, while the level of CD8 decreased, and CD4/CD8 ratio elevated. IgA and IgG levels of the treatment group were superior than those of the control group, and infection rate of the treatment group was lower than that of the control group, all with statistical significance (P 〈 0.05 ). Conclusions Being an immune modulator, LP could effectively improve the immune function of burn patients onboard oilers.
