Pringle肝血流阻断和半肝血流阻断在肝癌切除术中的应用比较 被引量:9

摘要 目的考察并比较Pringle肝血流阻断和半肝血流阻断在肝癌切除术中的作用。方法收集行肝切除术的肝癌病患168例,其中82例行半肝血流阻断法(HVC)切除肝脏,86例行Pringle法阻断入肝血流法切除肝脏。结果两组病例的年龄、性别比、肝硬化程度、肿块大小和位置等无显著差异(P〉0.05)。与HCV组比,Pringle组的阻断次数较多(P〈0.05),阻断时间、手术时间和出血量无显著差异(P〉0.05)。2个组别术前的肝功能指标均无显著差异(P〉0.05);与HVC组术前比,术后1~4d的血清白蛋白(Bumin)、血清胆红素(Bilimbin)和血清胆碱酯酶(Cholinesterase)无显著差异(P〉0.05),丙氨酸转氨酶(ALT)和碱性磷酸酶(ALP)显著增高(P〈0.05);与Pringle组术前比,术后1~4d的Bumin和Cholinesterase显著降低(P〈O.05),Bilirubin、ALT和ALP显著增高(P〈0.05);与HVC组术后比,Pringle组术后l~4d的Bumin和Cholinesterase显著降低(P〈0.05),Bilirubin、ALT和ALP显著增高(P〈0.05)。2个组别的术后并发症发生无显著差异(P〉0.05)。结论HVC和Pringle相比,HVC对肝功能的影响较小,略优于Pfingle法。
