摘要
目的探讨经皮肾镜气压弹道联合超声弹道碎石清石系统在肾及输尿管上段结石中的应用价值。方法选择曲靖市第一人民医院2011年10月至2013年1月泌尿外科上尿路结石患者142例作为研究对象,其中71例患者选择采用单纯经皮肾镜气压弹道清除结石,设为对照组,其余71例患者选择采用经皮肾镜气压弹道联合超声弹道碎石清石系统清除结石,设为实验组。分析并比较两组手术时间、术中出血量、术后并发症发生率、结石清除率。结果实验组的手术时间为(93.12±12.32)min、术中出血量为(50.32±15.67)ml、术后并发症发生率为4.2%,结石清除率为86.7%;对照组手术时间为(124.20±12.65)min、术中出血量为(90.23±17.81)ml、术后并发症发生率为15.5%,结石清除率为75.6%。实验组的手术时间、术中出血量、术后并发症发生率均少于对照组,结石清除率高于对照组,差异均有统计学意义(P<0.05)。结论经皮肾镜气压弹道联合超声弹道碎石清石系统对肾及输尿管上段结石清除率高,术后并发症少,值得在临床推广。
Objective To investigate the application of percutaneous nephroscopic pneumatic and ultrasound lithotripsy kiy- oishi system in renal and upper ureteral calculi. Methods One hundred and forty-two patients with upper ureteral calculi admit- ted in department of urology of our bospilal, from Oct. 2011 to Jan. 2013 were selected as objects, of which 71 patients were treated by percutaneous nephrolithotomy with pneumatic removing stones, as the control group, and the remaining 71 patients were treated with percutaneous nephroscopic pneumatic and ultrasound lithotripsy kiyoishi system clear stone, as test group. Op- eration time, intraoperative bleeding volume, postoperative complication rate and stone elarance rate were compared between the two groups. Results The operation time was (93.12 ± 12. 32 )rain, the intraoperative bleeding volume was (50. 32 ± 15.67) ml, the postoperative complication rate was 4. 2% , and the stone clearance rate was 86. 7% in test group; the operation time was (124. 20± 12. 65)min, the intraoperative bleeding volume was (90. 23± 17.81 ) ml, the postoperative complication rate was 15.5% , and the stone clearance tale was 75.6% in control group. The operation time, intraoperative bleeding volume and postoperative complication rate were less than that in the control group, and the stone clearance rate was higher than that in the control group, the differences were significant ( P 〈 0. 05 ). Conclusion Percutaneous nephroscopie pneumatic and ultra- sound lithotripsy kiyoishi system has high clearance rate of renal and upper ureteral calculi and less postoperative complications, so is worth to be clinical promoted.
