血清心肌酶、TNI及BNP水平变化对急性心肌梗死患者预后的影响 被引量:5

The effect of myocardial enzyme series,TNI and BNP levels changes on prognosis of patients with acute myocardial infarction

摘要 目的观察急性心肌梗死(AMI)患者血清心肌酶[天门冬氨酸转氨酶(AST)、乳酸脱氢酶(LDH)、乳酸脱氢酶同工酶-1(LDH-1)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)]、肌钙蛋白I(TNI)及脑钠肽(BNP)水平的变化,探讨其对预后的影响。方法选取我院AMI患者78例,用全自动生化分析仪测定血清AST、LDH、LDH-1、CK、CK-MB浓度;用西门子ADVIA Centaur xp测定TNI、BNP含量。结果与正常参考值比较,AMI患者血清AST、LDH、LDH-1、CK、CK-MB、TNI、BNP浓度水平均有不同程度升高;其中55例患者AST水平升高;48例患者LDH水平升高;48例患者LDH-1水平升高;51例患者CK水平升高;51例患者CK-MB水平升高;70例患者TNI水平升高;68例患者BNP水平升高。结论剧烈持久胸骨后疼痛、发热、白细胞、酶活性增高、心电图等变化,伴心律失常、休克、心力衰竭等为其临床特点,早期诊断及时治疗是降低其病死率关键,早期诊断及时治疗与预后密切相关,诊治过程中应控制血清心肌酶、TNI及BNP水平变化并加强护理,有助于提高治疗效果,改善预后,防止或延缓并发症发生。 Objective To study the level changes of myocardial enzyme series (AST, LDH, LDH - 1, CK, CK - MB), Tro- ponin I(TNI) and brain natriuretic peptide(BNP) in patients with acute myocardial infarction( AMI), and explore the influence on prognosis o AMI. Methods Seventy - eight AMI patients were collected in the hospital to detect serum AST, LDH, LDH - 1, CK, CK - MB levels with automatic biochemical analyzer, and the levels of TNI and BNP were determined with Siemens AD- VIA Centaur XP. Results Compared with reference values, serum levels of AST, LDH, LDH - 1, CK, CK - MB, TNI and BNP in AMI patients elevated to varying degrees. The results were as follows: 55 patients had AST increase, 48 patients had LDH increase, 48 patients had LDH -1 increase, 51 patients had CK increase, 51 patients had CK -MB increase, 70 patients had TNI increase, 68 patients had BNP increase. Conclusion Dramatic and lasting pain behind sternum, fever, increase of white blood cells and enzymes, the ECG changes, arrhythmia, shock, cardiac failure were clinical features AMI patients. Early diagnosis and timely treatment are the key to reduce the fatality rate and closely related to prognosis. The serum myocardial en- zyme series, TNI and BNP levels should be controlled and the AMI patients should be given special care, thus improving the treatment effect and revent or delay complications.
