摘要
目的通过分析我院2009年-2011年非淋球菌性泌尿生殖道感染者支原体检出及耐药性变异情况,为临床提供用药指导。方法采用法国生物梅里埃公司支原体培养、鉴定、计数及药敏试验试剂盒Mycoplasma IST2,回顾性分析我院3年中就诊患者的支原体培养及药敏试验结果。结果 3年中检测疑似泌尿生殖道感染患者4631例,支原体阳性率为39.4%~50.3%,呈逐年上升趋势;其中解脲脲原体(UU)检出率为27.0%~32.8%,人型支原体(MH)检出率为1.1%~1.4%,混合型(UU+MH)检出率为11.3%~16.1%。耐药监测结果,UU对四环素类敏感性没有明显的变化,大环内酯类、喹诺酮类变化非常显著;MH对原始霉素、交沙霉素、强力霉素、四环素敏感性高且较为稳定,对其余药物敏感性均较不理想;混合型对原始霉素、强力霉素、交沙霉素、四环素敏感性高,但后三者药物敏感性逐年略有降低。结论支原体感染呈逐年上升趋势,且耐药性变化也十分明显,临床医师治疗过程中应了解其流行病学及耐药性变化,合理使用抗菌药物。
Objective In order to provide guidance for rational drug use in clinic, mycoplasma detection and resistance variation were analyzed in patients with non - gonococeal urogenital tract infection from 2009 to 2011. Methods Myeoplasma IST2 kit from French BioM6rieux was used for culture, identification, counting and sensitive test of mycoplasma, and the test results of patients with non - gonococeal urethritis infection in three years （2009 - 2011 ） were retrospectively analyzed. Results In 3 years, 4631 cases were detected as suspected urogenital infection, and the positive rate of mycoplasma was 39.4% -50.3%,showing an increasing trend. Among the positive cases, the detection rates of ureaplasma urealyticum （ UU）, mycoplasma hominis（MH） and mixed （UU ＋ MH） were 27.0% - 32.8%, 1.1% - 1.4% and 11.3% - 16.1%, respectively. In the three years, UU showed no obvious changes to Tetracyeline in sensitivity, but significant to the Quinolones and Macrolides; MH showed high and stable sensitivity to pristinamycin, josamycin, doxycycline and tetracycline, but insensitive to other antibiotics.UU ＋ MH was sensitive to pristinamycin, doxycycline, josamycin and tetracycline, while the susceptibility of last three drugs decreased slightly year by year. Conclusion Mycoplasma infection showed an increasing trend, and the drug resistance dynamics was very obvious, so the clinician should be aware of its epidem^ology and resistance changes, ensuring the rational use of anti- biotics during the treatment process.
