摘要
目的 探讨慢性阻塞性肺疾病(COPD)稳定期患者药物治疗依从性及影响因素.方法 对100例COPD稳定期患者实施干预措施,定期随访,采用问卷调查法调查其药物治疗依从性,比较干预前后药物治疗依从性的差异,并分析其影响因素.结果 干预前COPD稳定期患者药物治疗依从性佳占34%,干预后依从性佳患者占73%,差异有统计学意义(x2=30.57,P<0.01).是否掌握吸入用药技术、是否独居、是否定期复查、是否有药物不良反应的患者药物治疗依从性不同,差异有统计学意义(x2分别为22.99,15.52,17.94,10.04;P <0.01);不同年龄、文化程度患者药物依从性不同,差异有统计学意义(x2分别为4.58,4.92;P <0.05).结论 针对不同的人群采用不同的干预措施,可提高COPD稳定期患者药物治疗依从性,减少急性加重的发生,提高其生活质量.
Objective To evaluate the medication compliance and its influencing factors in patients with stable chronic obstructive pulmonary disease (COPD). Methods The intervening measures were taken in 100 patients with stable COPD, with a regular follow-up. A questionnaire method was used to investigate the medication compliance of patients in this study. The difference of medication compliance was compared between before and after intervention, and the influencing factors were analyzed. Results There were 34% patients with good medication compliance before intervention, and 73% after intervention, the difference was statistically significant ( X2 = 30.57, P 〈 0.01 ). These factors which included inhalation technique mastery of medication, living alone, periodic review, and adverse drug reactions affected apparently medication compliance of patient, and these difference were statistically significant ( ~2 = 22.99,15.52,17.94, 10.04, respectively; P 〈 0.01 ) between "yes" and "no" about above factors; also the medication adherence were different in patients with different age or educational level, and the difference was statistically significant (X2 = 4. 58, 4. 92, respectively; P 〈 0.05 ). Conclusions The different intervening measures should be used in patients with stable COPD according to different patients' characteristics, which can enhance their medication compliance, reduce the frequency of exacerbations and improve their quality of life.
