摘要
目的:了解癌症疼痛患者出院后的疼痛管理现状,并分析其用药行为的影响因素,为临床提高出院癌症疼痛患者用药依从性、保证疼痛治疗的有效性提供依据。方法:通过标准化的电话随访,对202例癌症患者在出院后按照统一的随访流程与问卷调查了解患者出院后的疼痛控制现状、用药行为、用药副反应以及用药顾虑。结果:肿瘤患者出院后发生过中重度疼痛的22例(10.89%);未遵医嘱用药24例(11.88%);发生至少一种药物副反应96例(47.52%);存在用药顾虑33例(16.34%)。未遵医嘱用药的患者中发生中重度疼痛的比例较高(P<0.05),发生了药物副反应的患者遵医嘱用药行为较差(P<0.05),有用药顾虑的患者遵医用药行为较差(P<0.05)。结论:癌症疼痛患者出院后的用药行为受到药物副反应与用药顾虑的影响,在对患者进行阿片类药物止痛治疗的同时应当预防性使用预防便秘与呕吐的药物,并加强对患者的疼痛教育,提高患者疼痛自我管理水平,维持出院后疼痛治疗的效果。
Objective：To understand the current situation of pain management among patients with cancer pain,and analyze the factors that might affect the painkillers use behavior,which would be useful in enhancing the painkillers use compliance and ensure the effect of pain treatment for the basis.Methods：The study got information about the pain,painkillers use behavior,and side effects,and the concerns about taking painkillers of the patients,through standardized telephone follow-up using standardized questionnaire,and the statistics were described and analyzed using chi-square test.Results：There were 22 cases （10.89％） reported to have experienced moderate to severe pain post-discharge,and 24 cases （11.88％） didn＇t take the painkillers regularly as the doctors told,while 96 cases （47.52％） complained about undergoing at least one side effects of the medication,and 33 cases （16.34％） admitted to haveconcerns about using painkillers.The rate of having experienced moderate to severe pain was higher among the patients failed to use painkillers regularly （P ＜ 0.05）,rate of taking painkillers regularly was lower both among patients who experienced at least one side effect （P ＜ 0.05）,and among patient with concerns about painkillers （P ＜0.05）.Conclusion：Painkillers use behavior was affected by the side effects and concerns about the pills among patients with cancer pain.While the doctors prescribe painkillers for patients,medications to prevent constipation and nausea should also be prescribed at the same time.The clinical nurses should pay attention to patient education about pain management to improve the self management of pain among the patients,in order to keep the effectiveness of pain treatment after the patients discharge from hospital.
