摘要
目的评价支架成形术治疗症状性大脑中动脉M1段狭窄的疗效及安全性。方法51例症状性大脑中动脉Ml段狭窄患者接受支架成形术治疗,术后随访1—3a,观察患者手术后缺血性卒中改善程度,术前、术后和随访时血管狭窄程度,术后手术相关性并发症发生情况。结果5l倒大脑中动脉M1段狭窄患者共放置36枚球扩式支架,15枚自膨式支架。19例行预扩张。手术成功率98%。9例患者伴有一侧椎动脉起始部重度狭窄并同时置入9枚球扩式支架,7例患者合并一侧颈内动脉起始部中重度狭窄,其中4例同时置入4枚自膨式颈动脉支架。术前狭窄率为(86.3±11.6)%,术后剩余狭窄率(9.7±4.4)%。与术前相比,症状消失与改善47例,症状改善率94%。无严重神经功能恶化及死亡病例发生。结论支架成形术治疗症状性大脑中动脉M1段狭窄是改善缺血性脑血管病患者神经症状的安全、有效方法,短中期随访临床疗效满意。
Objective It is to evaluate the therapeutic effect and safety of angioplasty and stenting in the M1 part of symp- tomatic middle cerebral artery stenosis. Methods 51 patients with symptomatic middle cerebral artery stenosls in the M1 part were treated with angioplasty and stenting placement. The patients were followed up for 1 -3 years, and ischemic stroke, pre- operative, postoperative and follow-up of vessel stenosis, stent related complications after operation were observed. Results 51 patients were placed 36 balloon expandable stent, 15 self expandable stents, 19 cases of pre expansion. Operation success rate was 98%. 9 patients with severe unilateral vertebral artery stenosis in the initial part were placed 9 balloon expandable stent. 7 patients had severe or moderate unilateral internal carotid artery stenosis in the initial part, in which 4 ones were place 4 self expanding carotid stent at the same time. The mean preoperative stenosis rate was (86.3 ±1.6)% , the rate of residual steno- sis after operation was (9.7 ±4.4) %. Compared with that before operation, the symptoms disappeared and improved in 47 ea- ses, the symptom improvement rate was 94 %. There was no severe neurological deterioration and death cases. Conclusion Angioplasty and stenting in the M1 part of symptomatic middle cerebral artery stenosis is safe and efficacy, which can improve neurologie outcome and clinical outcome in selected patients , and its short and medium term of following up results were satis- factory.
