摘要
[目的]探讨适用于全身麻醉开胸术后病人理想的氧气雾化吸入治疗时间。[方法]将120例肺癌手术病人随机分成4组,A组术后采用常规氧气雾化吸入,每日2次;B组、C组、D组分别以持续6h、持续12h、持续24h采用改良滴注式面罩氧气雾化吸入,3d后评价效果。[结果]B组、C组、D组排痰效果显著优于A组,肺部并发症发生率也低于A组(P<0.01),但B组、C组、D组三组间差异无统计学意义(P>0.05);B组、C组、D组血气分析、动脉血氧饱和度各观察指标优于A组(P<0.05),但三组间差异无统计学意义(P>0.05);4组病人呼吸频率变化差异无统计学意义(P>0.05)。[结论]全身麻醉开胸术后病人每日氧气雾化吸入治疗时间达6h以上,能有效帮助排痰,改善肺功能,减少肺部并发症。
Objective：To probe into ideal oxygen inhalation therapy time for general anesthesia patients after thoracotomy. Methods. A total of 120 ca- ses of lung cancer patients undergoing surgery were randomized into 4 groups,A group of patients used conventional oxygen inhalation,2 times a day B group, C group, D group of patients respectively received last 6 h, 12 h,24 h improved dripstyle mask oxygen inhalation, then to evaluate the effect after 3 days. Results： The expectoration effect in group B,group C and group D was significantly better than that in group A,the incidence of pulmonary complications was also lower than that in group A（P〈0.01）, but there was no statistically significant difference among the three groups （P〈0.05） ; the observed indicators of blood gas analysis and arterial oxygen saturation of group B,group C and group D were better than that in group A（P〈0. 05）, but no statistically significant difference among the three groups（P〈0.05） ; there was not statistically significant difference in breathing frequency among 4 groups of patients （P〈0. 05）. Conclusion： daily oxygen inhalation therapy time for patients after thoracotomy under general anesthesia is up to 6 h or more,can effectively help expectoration, improve lung function and reduce pulmonary complications.
