摘要
目的分析腹腔镜下保胆取石在治疗胆囊结石中的临床应用。方法回顾2009年3月至2013年3月实施腹腔镜下保胆取石治疗胆囊结石108例患者的临床资料，纳入的患者有严格的入选标准，通过随访了解腹腔镜下保胆取石术优点和不足。结果108例胆囊结石患者行腹腔镜下保胆取石手术均成功，无中转开腹或切除胆囊，手术时间（82．0±10．0）min，出血量在（26．0±12．0）ml。结石在1-32枚不等，共取出结石238枚。出院前切口一期愈合，全部患者出院前超声检查胆囊内无残余结石，肝功能正常。98例患者术后门诊或电话得到随访，随访率90．7％，随访时间3个月～4年，其中3例患者于手术后1年、2年后胆囊结石复发，复发率3．1％。结论经腹腔镜保胆取石术简单、安全、有效，在严格把握手术指征的前提下，是治疗胆囊结石的一种值得推广的微创治疗方法。
Objective To investigate the clinical application of laparoscopic gallbladder-preserving choleeystolithotomy in treating cholecystolithiasis. Methods From March 2009 to March 2013, the clinical data of 108 patients with cholecystolithiasis who had undergone laparoseopic gallbladder-preserving cholecystolithotomy were analyzed retrospeetively. Postoperative follow-up was performed to determine the advantages and disadvantages of laparoscopic gallbladder-preserving eholeeystolithotomy. Results The 108 patients underwent successfully laparoscopie gallbladder-preserving cholecystolithotomy, without intraoperative conversion or removal of the gallbladder. Operation time was （82.0 ± 10.0） minutes and the amount of bleeding was （ 26.0 ± 12.0 ） mt. The number of stones varied from 1 to 32 per gallbladder, and a total of 238 stones were removed. All of the patients healed without residual stones and showed normal liver function. 98 patients （90.7%） were followed up for 3 months to 4 years. Three patients（3. 1% ） had recurrence of cholecystolithiasis 1 or 2 years after operation. Conclusion Laparoseopie eholecystotomy is simple, safe, and effective and is worth popularizing in minimally invasive treatment of cholelithiasis.
