摘要
目的:分析2型糖尿病(T2DM)微血管病变(DMAP)患者同型半胱氨酸(Hcy)、叶酸(FA)及维生素B12(VitB12)血液水平变化及意义。方法:115例T2DM患者分为无微血管病变组(NDC组,n=41)、视网膜病变组(DR组,n=31)和肾病组(DN组,n=43),选择50例体检健康者作为对照组。平行检测各组血清Hcy、FA、VitB12水平并分析其相关性;同时观察高Hcy血症T2DM患者治疗前后以上指标变化。结果:T2DM各组血清Hcy水平高于对照组,FA、VitB12水平低于对照组(P<0.05或P<0.01)。DR组和DN组血清Hcy水平高于NDC组,FA、VitB12水平低于NDC组(P均<0.05)。T2DM患者血清FA、VitB12水平与Hcy水平呈显著负相关(r=-0.487;r=-0.399,P均<0.01)。高Hcy血症T2DM患者治疗后Hcy水平较治疗前明显降低(P<0.01),FA、VitB12水平明显升高(P<0.01)。结论:DMAP患者Hcy、FA、VitB12水平异常比单纯T2DM更为严重,这可能是引起微血管病变的重要因素。
Objective:To analyze the variations of blood concentrations of homocysteine(Hcy),folic acid(FA)and vitamin B12(VitB12)in patients with type 2diabetes mellitus(T2DM)microangiophathy(DMAP)and evaluate their clinical significances.Method:115cases of T2DM patients were divided into three groups:no-diabetic complication group(NDC,n =41),diabetic retinopathy group(DR,n=31),and diabetic nephropathy group(DN,n=43).A group of 50healthy people was regarded as control group.Measured the levels and the correlations of blood concentrations of Hcy,FA,and VitB12 for different groups.Measured the above-mentioned parameters for patients with high Hcy level before and after medical treatments.Results:All three T2DM groups showed higher Hcy levels and lower FA and VitB12levels(P<0.05or P<0.01)compared to the control group.DR and DN groups showed higher Hcy levels and lower FA and VitB12 levels(average P<0.05)compared to NDC group.Hcy level was strongly negative-correlated to FA and VitB12levels(r=-0.487,r=-0.399,P<0.01).There were significant decreased in the Hcy levels(P<0.01)and significant increased in the FA and VitB12 levels(P<0.01)for patients with T2DM after medical treatments.Conclusion:The abnormal variations of Hcy,FA,and VitB12 levels in patients with DMAP are more severe than patients with only T2DM,which is a possible cause for microangiophathy.
