经阴道三维能量多普勒超声诊断绝经后阴道出血妇女子宫内膜疾病的价值研究 被引量:3

摘要 目的:探讨经阴道三维能量多普勒超声对绝经后阴道出血妇女子宫内膜疾病的诊断及鉴别诊断价值。方法:对2010年6月~2012年6月间在该院就诊的绝经后阴道出血妇女进行筛选得到115例,分为子宫内膜良性病变组64例(良性组)、子宫内膜癌51例(恶性组)。行三维多普勒超声检查,测量病变部位内膜血管的搏动指数(PI)、阻力指数(RI)、收缩期峰值血流速度(PSV),计算出血管指数(VI)。并对病变组织进行免疫组化检测,计数微血管密度(MVD)。结果:超声血管参数在两组患者中差异均有统计学意义(P〈0.05或P〈0.01);ROC曲线显示,以VI1.61条/cm3为鉴别诊断临界点,特异度和敏感度分别为81.10%、94.92%;VI与MDV之间呈正相关;VI在不同FIGO分期间差异有统计学意义(P〈0.05或P〈0.01)。结论:经阴道三维能量多普勒超声可以在术前准确反映出子宫内膜病灶血流情况,对于良、恶性病变的鉴别诊断也有一定的价值。
