摘要
根据2008年全国老年人口健康状况调查(CLHLS),选取Stepwise和Logistic两种回归模型实证分析医疗保障对我国65岁以上老年人医疗服务需求的影响,得出以下结论:第一,医疗保障显著促进了老年人对医疗服务的需求,使其家庭医疗负担得以降低、及时就医率得以提升。虽然大病保险这一保障形式没有通过显著性检验,但其回归方向较为稳定。第二,不同的医疗保障政策对老年人医疗服务需求的促进效果有所差异。医疗保障在提升老年人及时就医率上所起的作用基本是均匀的,但在降低家庭医疗负担方面,合作医疗发挥的效果最差。第三,医疗保障对老年人医疗服务需求利用的影响存在显著的城乡差异。医疗保障在城镇地区发挥的效果要高于农村地区,农村老人未能及时就医的比率明显低于城镇老人。最后在研究结论的基础上提出加快完善补充医疗保险,提升老年人参与商业医疗保险的意愿、加大公共财政对合作医疗的投入力度、适度增加合作医疗在慢性病方面的医疗服务以及医疗保障政策的灵活性等建议。
According to the national health status data of the elderly in 2008. We select two models of stepwise and Logistic regression to empirical analysis how medical security impact on demand for medical services for the people over 65 years old, and draw a conclusion as follows: first, medical security has a crowding-out effect on demand for medical services. Then reduce the family medical burden of elderly and promote the timely medical treatment rate on the overall. Although illness insurance did not pass the significance test, but the return direction is stable. Second, medical security in improving timely medical treatment rate of elderly have the uniform effects on the basic, but in reducing family medical burden, cooperative medical has the worst effect. Thirdly, medical insurance for medical care demand of the elderly has a significant difference between urban and rural areas. The ratio of timely medical treatment rate in the rural was significantly lower than urban people. And the ratio of no timely medical treatment in rural areas because of economic reasons is much higher than that of the urban elderly. On the basis of this conclusion, proposed thinking of to accelerate the improvement of supplementary medical insurance, and improve the consciousness of elderly to participate in commercial medical insurance, and increase investment of public finance on the cooperative medical, and a moderate increase in chronic disease care of cooperative medical and medical policy flexibility and so on.
