摘要
目的了解2010-2011年耐甲氧西林金黄色葡萄球菌(MRSA)SCCmec基因分型及PVL基因携带情况并对其耐药谱进行分析。方法收集2010年和2011年从临床各标本所分离出的MRSA,分别为54株和119株,应用多重PCR方法对MRSA进行SCCmec基因分型,PCR扩增PVL基因,采用MicroScanWalkAway-40全自动微生物鉴定药敏分析仪进行细菌的鉴定及药敏试验,部分药敏试验采用K-B法。结果 2010年54株MRSA中SCCmecⅡ型42株76.26%,SccmecⅢ型11株20.37%,未分型1株1.88%,2011年119株MRSA中SCCmecⅡ型86株72.27%,SCCmecⅢ型28株23.53%,SCCmecⅣ型3株2.52%,其中SCCmecⅣc型2株,SCCmecⅣd型1株,未分型2株1.68%,在2011年两株SCCmecⅣc型MRSA中检测到PVL基因,其余菌株均未检测到PVL基因;SCCmecⅡ型和SCCmecⅢ型MRSA 2010年和2011年对万古霉素、磺胺甲噁唑/甲氧苄啶、利奈唑胺、喹奴普汀/达福普汀的耐药率均为0,对氯霉素和利福平的耐药率均<30.0%,SCCmecⅢ型MRSA对四环素的耐药率由2010年的36.36%上升至2011年的78.57%(P<0.05),两种SCCmec基因型的MRSA均表现为多药耐药性。结论 2010-2011年临床分离的MRSA以SCCmecⅡ型为主,SCCmecⅡ型和SCCmecⅢ型MRSA对抗菌药物呈多药耐药;2011年大连地区开始出现SCCmecⅣ型MRSA,并且携带PVL基因。
OBJECTIVE To understand the SCCrnec genotyping of methicillin-resistant Staphylococcus aureus （MRSA） and its carrying PVL gene and analyze the drug resistance spectrum from 2010 to 2011. METHODS The pathogenic MRSA strains isolated from various specimens in clinical were collected, there were 54 strains detected in 2010, 119 strains detected in 2011, then the SCCmec genotyping for MRSA was performed by multiplex PCR, the PVL gene was amplified by PCR,the MicroScanWalkAway-40 automatic microorgnisms analyzer was used for strain identification and drug susceptibility testing, and drug susceptibility testing for some of the strains was performed with the use of K-B method. RESULTS A total of 54 strains of MRSA isolated in 2010, among which there were 42 （76.26%） strains of MRSA with SCCmec type Ⅱ, 11 （20.37%） strains of MRSA with SCCmec type Ⅲ, and 1 （1.88%） strain with genotype unknown, among the 119 strains of MRSA were isolated in 2011, there were 86 （72.27） strains of MRSA with SCCrnec type Ⅱ , 28 （23.53%） strains of MRSA with SCCmec type Ⅲ , 3 （2.52%） strains with SCCmec type Ⅳ, including 2 strains with SCCmec type Ⅳc and 1 strain with SCCrnec type Ⅳd, and 2 （1.68%） strains of MRSA with genotype unknown. Among the MRSA strains isolated in the past two years, there were only 2 strains of MRSA with SCCmec type IV c isolated in 2011, the PVL gene has been only detected in two strains and has not been detected in the rest of strains. The drug resistance rates of the MRSA strains with SCCrnec type Ⅱ or SCCmec type Ⅲ to vancomycin, sulfamethoxazole-trimethoprim, linezolid, and quinupristin-dalfopristin were 0, the drug resistance rates to chloramphenicol and rifampicin were less than 30.0%, and the drug resistance rate of the MRSA strains with SCCmec type HI to tetracycline increased from 36.36% in 2010 to 78. 57% in 2011 （P〈0.05）, and the MRSA strains with both SCCmec genotypes were multidrug-resistant.
