摘要
【目的】在中医基础理论的指导下,针对气阴两虚型消渴患者的饮食宜忌,建立气阴两虚型消渴病的中医饮食治疗方案,探讨该方案对消渴病患者的影响效果。【方法】将60例气阴两虚型消渴病患者随机分为中医治疗组和对照组各30例,2组均予糖尿病基础治疗及护理。对照组采用常规的饮食治疗方案,中医治疗组采用中医饮食治疗方案进行护理干预90 d。比较2组治疗前后的空腹血糖、餐后血糖、糖化血红蛋白水平及生存质量的变化。【结果】(1)治疗后2组患者的空腹血糖、餐后2 h血糖、糖化血红蛋白水平均较治疗前下降,差异有统计学意义(P<0.05),且中医治疗组的下降作用有优于对照组趋势,但差异无统计学意义(P>0.05)。(2)治疗后中医治疗组的生存质量(DSQL)特异性量表4个方面评分均较治疗前显著改善(P<0.05),而对照组仅糖尿病对社会关系的影响、治疗对糖尿病患者的影响2个方面评分显著改善(P<0.05),治疗后组间比较,中医治疗组在生理功能、心理/精神、对患者的影响等3个方面的改善显著优于对照组,差异有统计学意义(P<0.05)。【结论】中医饮食治疗方案能有效降低气阴两虚型消渴病患者的空腹血糖、餐后血糖和糖化血红蛋白,提高临床疗效,改善糖尿病患者的生存质量。
Objective To investigate the therapeutic effect of traditional Chinese medicinal diet therapy (TCMDT), which was established on the basis of traditional Chinese medical theory, for the treatment of diabetic patients with deficiency of both qi and yin. Methods A total of 60 diabetes mellitus (DM) patients with deficiency of both qi and yin which was in accordance with diagnostic and inclusion criteria were divided into TCM-DT treatment group and control group according to the randomized, controlled and single-blind design. Each group had 30 cases, and both groups were given DM basic treatment and nursing. The control group was given conventional diet therapy, while the TCM-DT treatment group was given nursing and intervention according to TCM-DT. The treatment lasted for 90 days, and the levels of fasting blood glucose (FBG), 2-hour postprandial blood glucose (2hPBG) and glycosylated hemoglobin as well as the diabetes specific quality of life (DSQL) in two groups were compared before and after treatment. Results (1)FBG, 2hPBG and glycosylated hemoglobin of the both groups after intervention were all decreased compared with those before treatment (P〈 0.05), and the decrease of TCM-DT treatment group showed a trend of being superior to that of the control group (P〉0.05). (2) The scores of four scales of physical functioning (PF), mental heahh(MH), social Functioning (SF) and impact on patients (IOP) of DSQL scale were improved in TCM-DT treatment group after treatment (P〈0.05 compared with those before treatment), while the control group only had effect on improving the scores of SF and IOP (P〈0.05). The improvement of PF, MH and IOP in TCM-DT treatment group was superior to that in the control group (P〈0.05) . Conclusion The TCM -DT is effective on reducing the levels of FBG, 2hPBG and glycosylated hemoglobin in diabetic patients with deficiency of both qi and yin, and on improving the clinical efficacies and DSQL of the patients.
