磷酸酰肌醇3激酶/哺乳动物雷帕霉素靶蛋白信号通路激活与乳腺癌临床病理相关性

Correlation between phosphoinositide 3-kinase/mammalian target of rapamycin signaling pathway activation and clinicopathology in breast cancer

摘要 目的探讨磷酸酰肌醇3激酶(PI3K)/哺乳动物雷帕霉素靶蛋白(mTOR)信号通路与乳腺癌临床病理特征的相关性。方法采用免疫组织化学法检测42例乳腺病、88例乳腺癌患者的手术切除组织中PI3K、mTOR和磷酸化mTOR(p-mTOR)的表达,分析其与各临床病理特征之间的关系。结果乳腺癌组织中PI3K、mTOR、p-mTOR蛋白阳性表达率均显著高于乳腺病组织中(P值均<0.05)。乳腺原位癌组织中PI3K、mTOR蛋白阳性表达率与乳腺浸润癌组织中的差异均无统计学意义(P值均>0.05),乳腺原位癌组织中p-mTOR蛋白阳性表达率显著低于乳腺浸润癌组织中(P<0.05)。组织学分级Ⅰ、Ⅱ级者的p-mTOR蛋白阳性表达率均显著低于组织学分级Ⅲ级者(P值均<0.05),组织学分级Ⅰ级者的mTOR蛋白阳性表达率显著低于组织学分级Ⅲ级者(P<0.05);有淋巴结转移者的p-mTOR蛋白阳性表达率显著高于无淋巴结转移者(P<0.05);有脉管癌浸润者的p-mTOR蛋白阳性表达率显著高于无脉管癌浸润者(P<0.05);Cerb-B2阳性者的p-mTOR蛋白阳性表达率显著高于Cerb-B2阴性者(P=0.028)。乳腺良恶性病变组织中PI3K与mTOR(r=0.818)、PI3K与p-mTOR(r=0.707)、mTOR与p-mTOR(r=0.863)之间均呈正相关(P值均<0.05)。结论 PI3K/mTOR信号导通路可能在介导乳腺癌的发生、发展过程中起到重要作用,p-mTOR可能成为新的乳腺癌进展预测因子。 Objective To discuss the correlation between phosphoinositide 3-kinase(PI3K)/mammalian target of rapamycin (mTOR) signal pathway and clinical pathological parameters in breast cancer tissue. Methods The expression of PI3K, mTOR and p-mTOR in 42 specimens of benign breast disease and 88 specimens of breast cancer was detected by the immunohistochemistry method, and then its relationship with clinical and pathological factors was analyzed. Results The expression of PI3K, mTOR and p-mTOR in breast cancer was higher than in benign breast disease, and the difference was statistically significant (all P〈0.05). The positive rates of PI3K and mTOR in carcinoma in situ and invasive carcinoma showed no significant difference (both P〉 0.05). The positive expression rate of p-mTOR in carcinoma in situ was significantly lower than that in invasive carcinoma ( P〈0.05). In invasive carcinoma, the positive rates of mTOR and p-mTOR increased with the increase of pathological grades (both P〈0.05). The strong expression rates of p-mTOR in lymphnode metastasis, vessel invasion and CerbB-2 overexpression were significantly higher than that in non-lymphnode metastasis, non vessel invasion and non-CerbB-2 expression (all P〈0.05). There was a positive correlation among PI3K, mTOR and p-mTOR expression in benign and malignant breast tissues ( r = 0. 818, 0. 707, 0. 863, all P 〈 0. 05).Conclusion PI3K/mTOR signal pathway may play an important role in the occurrence and development of breast cancer. The p-mTOR may be served as a new predictor of tumor progression.
