摘要
【目的】探讨体外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)术后的PCOS患者和输卵管性不孕患者受孕后的妊娠结局。【方法】选取2009年1月至2012年12月在中山大学附属第一医院生殖中心第一次行IVF的PCOS患者共104例,对照组为第一次行IVF的输卵管性不孕患者共1 658例。两组患者按年龄均分为<35岁组和≥35岁组。对4组患者的基本资料、IVF-ET周期的卵子和胚胎情况以及妊娠结局进行回顾性比较。【结果】PCOS患者IVF周期中的临床妊娠率、流产率、早产率、多胎率、大于胎龄儿、小于胎龄儿以及畸形率与同年龄组的对照组比较,差异没有统计学意义(P>0.05),但是<35岁PCOS患者妊娠期糖尿病以及妊娠期高血压疾病的发生率高于对照组,差异有统计学意义(P<0.05),而在≥35岁PCOS患者该两种疾病的发生率与对照组相比,差异不具有统计学意义(P>0.05)。【结论】<35岁PCOS患者行IVF-ET治疗后的妊娠期糖尿病、妊娠期高血压疾病的发生率增加,≥35岁PCOS患者的各种并发症发生率未升高。
[Objective] To investigate the pregnancy outcome in different age women with polycystic ovary syndrome （PCOS） compared with control groups after in vitro fertilization and embryo transfer （IVF-ET）. [ Methods ] Women with PCOS who underwent the first time of IVF from February 2009 to November 2011, were considered. A total of 104 PCOS patients （PCOS group） and 1658 patients with tubal infertility （control group） were included. They were divided into four groups： 〈 35 years PCOS group, /〉 35 years PCOS group, 〈 35 years tubal factor group, /〉 35 years tubal factor group, and reviewed focusing on the basal characteristics, IVF- ET outcomes, and pregnancy outcomes. [ Results ] The clinical pregnancy rate, miscarriage rate, preterm delivery rate, multiple pregnancy rate, large for gestational age （LGA） infant and small for gestational age （SGA） infant rate, malformation infant rate were not different between two groups at the same age stage （P 〉 0.05）. On the other hand, the morbidity of gestational diabetes mellitus （GDM） and hypertensive disorder complicating pregnancy （HDCP） was significantly higher than those in control group in 〈 35 years group （P 〈 0.05） but was not significantly higher in the ≥35 years group （P 〉 0.05）. [Conclusion] PCOS patients after IVF-ET did not present various higher pregnancy outcomes, but 〈35 years PCOS patients presented some worse pregnancy complications, especially in GDM and HDCP.
