摘要
目的:观察加味大柴胡汤联合活瓣式囊肿-胃内引流术治疗老年假性胰腺囊肿的临床疗效。方法:将116例患者随机分为观察组、对照组各58例。2组均采用活瓣式囊肿-胃内引流术治疗;观察组于入院诊断明确即加用加味大柴胡汤,1剂/d,连续服用15天。结果:痊愈率观察组为77.59%,对照组为55.17%,2组相比差异显著(P<0.05)。总有效率观察组为89.66%,对照组为67.24%,2组相比差异显著(P<0.05)。纳差、腹胀、恶心、呕吐、腹水等临床症状积分、血尿淀粉酶术后第3天与入院时比较,差异显著(P<0.05);术后第3天2组组间比较,差异显著(P<0.05);对照组手术前与入院时比较无显著差异(P>0.05);观察组手术前与入院时比较,有显著差异(P<0.05)。观察组复发1例(1.72%),对照组复发6例(10.34%),2组复发率比较差异显著(P<0.05)。结论:加味大柴胡汤联合活瓣式囊肿-胃内引流术治疗老年假性胰腺囊肿临床疗效显著,可较快减轻临床症状,降低血尿淀粉酶水平。
Objective：To observe therapeutic effects of modified DaChaiHuTang and valve type cyst-gastric drainage in treating senile patients with pancreatic pseudocyst. Method：All 116 patients were randomized into the observation group and the control group. Both groups received valve type cyst-gastric drainage; the observation group also took modified DaChaiHuTang, one dose per day for 15 days consecutively. Result：Cure rate of the obser-vation group was 77.59%, higher than 55.17%of the control group with significant difference （P〈0.05）. Total effec-tive rate of both groups were 89.66%and 67.24%, the comparison between both groups showed significant differ-ence （P〈0.05）. the difference was significant when clinical symptom scale of anepithymia, abdominal distention, nausea, vomiting, ascites and hematuria amylase after surgery at the 3rd day were compared with those at hospital-ized time （P〈0.05）;the comparison between both groups at the 3rd day after surgery demonstrated significant differ-ence （P〈0.05）;the comparison of the control group between before surgery and hospitalized time had no significant difference （P〉0.05）;it had significant difference of the observation group （P〈0.05）. There were one case relapse in the observation group （1.72%） and six cases in the control group （10.34%）, the comparison between both groups in relapse rate showed significant difference （P〈0.05）. Conclusion：Modified DaChaiHuTang and valve type cyst-gas-tric drainage are effective in treating senile patients with pancreatic pseudocyst, the treatment could relieve clinical symptom rapidly and decrease the level of hematuria amylase.
