摘要
目的:观察分析壶腹部周围癌行胆胰十二指肠结合部切除治疗的临床疗效,总结其临床适应症和临床经验。方法:选取我院2009年5月至2011年5月79例壶腹部周围癌的患者,其中采取胆胰十二指肠结合部切除治疗有48例,设为A组;行局部肿瘤切除术治疗有20例,设为B组;行姑息性手术有11例,设为C组,观察比较三种治疗方式的疗效、术后并发症发生率及复发率。结果:三组疗效比较存在明显差异(P<0.05),具有统计学意义。结论:壶腹部周围癌行胆胰十二指肠结合部切除治疗的临床疗效显著,能够显著地降低术后并发症,减少远期复发,其手术难度及手术风险皆明显下降,安全可靠,值得临床上推广应用。
Objective： To observe and analyze the clinical efficacy of periampullary cancer through the resection of gall duodenal conjunction and summarize the clinical indications and clinical experience.Methods： In our hospital,from May 2009 to May 2011,there were 79 cases of periampullary cancer patients.The duodenal junction resection was for 48 as group A;partial resection of the tumor was for 20 as group B;palliative surgery was for 11 as group C.The efficacy,the incidence of postoperative complications and recurrence rate of the three treatments were observed and compared.Results： The efficacy of the three groups is significantly different（P〈0.05） with statistical significance.Conclusion： The resection of gall duodenal conjunction for periampullary cancer has significantly clinical effect.It can obviously reduce postoperative complications and the long-term recurrence.The surgical difficulty and risk are significantly decreased.Such treatment is safe,dependable and worth promoting vigorously.
