摘要
目的探讨医院-居家康复训练对首发分裂症患者康复的作用。方法将104例首发分裂症患者随机分为干预组和对照组,两组患者在住院期间对其患者及家属进行同样的精神分裂症知识健康教育,患者出院时干预组的患者由护士-患者-家属共同讨论商订个体化的医院-居家康复训练计划。内容涉及家庭布置、作息及服药时间安排,清理家庭卫生、个人生活用品采购,访客接待等。由患者具体实施,家属提供支持和督导落实,护士每个月进行电话随访,对康复过程中存在的问题共同分析原因达成共识后,改进调整训练计划,护士持续跟进的指导。对照组不进行特殊干预。由专职人员采用BPRS及SDSS量表在两组患者出院时、6个月时、12个月时各评定一次。结果两组患者在出院6个月时BPRS、SDSS测评结果及复发率差异无统计学意义(P>0.05),在12个月时,干预组BPRS、SDSS测评结果及复发率较对照组差异具有统计学意义(P<0.05)。结论医院-居家康复训练能提高首发分裂症患者及家属的遵医行为,促进患者社会功能的改善,降低复发率,对首发分裂症患者的康复具有重要的作用。
Objective To investigate the effect of hospital-home rehabilitation training on patients with first-episode schizophrenia. Methods A total of 104 patients with first-episode schizophrenia were randomly di- vided into the intervention group and the control group. During the hospitalization, the patients and their families received the same health education about schizophrenia. When the patients of intervention group were discharged, the nurses, patients and their families discussed and formulated individualized hospital-home rehabilitation train- ing program, which involves family arrangement, schedule and timing of the medication, clean up household, per- sonal supplies purchase, and reception of visitors. The implementation was supervised by their families and the nurses followed up by telephone monthly. The problems in the rehabilitation process were analyzed to adjust the training plan. The BPRS, SDSS scale were used to evaluate the two groups of patients at discharge, 6 months, 12 months after discharge. Results There was no statistically significant difference between the two groups six months after discharge in the BPRS and SDSS evaluation and the recurrence rate （/9〉0.05）. There was statistically significant difference between the two groups 12 months after discharge in the BPRS and SDSS evaluation and the recurrence rate （P〈0.05）. Conclusion The hospital-home rehabilitation training can improve the compliance of the patients with first-episode schizophrenia and their families, promote the social function of the patients, and reduce the recurrence rate.
