Application of MR Perfusion Weighted Imaging at 3. 0 T in Diagnosing Primary Central Nervous System Lymphoma and High-Grade Glioma

摘要 目的探讨灌注加权成像(PWI)在原发性中枢神经系统淋巴瘤(PCNSL)与高级别胶质瘤(HGG)中的鉴别诊断价值,分析PWI的信号强度-时间曲线峰值和信号恢复率与相对脑血容量(rCBV)的相关性。方法 22例PCNSL与30例HGG患者均采用Siemens Verio 3.0 T磁共振扫描仪行常规MR平扫、PWI及增强扫描。测量PCNSL与HGG病灶信号强度-时间曲线的峰值、信号恢复率和rCBV,应用两样本t检验及直线回归分析数据。结果PCNSL峰值、信号恢复率和rCBV的均数分别为242.86±123.16、1.54±0.57和1.53±0.70,HGG峰值、信号恢复率和rCBV的均数分别为466.35±132.49、0.69±0.20和5.52±1.58,峰值、信号恢复率和rCBV在PCNSL与HGG的鉴别诊断中差异均有统计学意义(P<0.05)。PCNSL的峰值与rCBV显著正相关(r=0.82,P<0.05);信号恢复率与rCBV显著负相关(r=-0.73,P<0.05)。HGG的峰值与rCBV无显著相关性(r=0.36,P=0.05);信号恢复率与rCBV无显著相关性(r=-0.12,P>0.05)。结论 PWI能准确地鉴别诊断PCNSL与HGG。PCNSL的峰值和信号恢复率与rCBV有良好的相关性,HGG的峰值和信号恢复率与rCBV无相关性。 Objective To investigate the value of PWI in the differential diagnosis of primary central nervous system lymphoma (PCNSL) and high-grade glioma (HGG). To analyze the correlation between high peak (PH) of signal intensi- ty-time curve and relative cerebral blood volume ( rCBV ) , percentage of signal intensity recovery (PSR) and rCBV. Methods Twenty two patients with PCNSL and 30 patients with HGG underwent conventional magnetic resonance imaging and perfusion weighted imaging on 3.0 T using the SIEMENS VERIO scanner. PH, PSR and rCBV were obtained in both PCNSL and HGG. Two-sample t test and linear regression were used for statistic analysis. Results The PH and rCBV values in PCNSL were significantly lower than those in HGG (242.86 ± 123.16 vs 466.35± 132.49, P 〈 0.05 ; 1.53 ±0.70 vs 5.52 ± 1.58, P 〈0.05 ). The PSR in PCNSL were significantly higher than that in HGG ( 1.54 ±0.57 vs 0.69 ± 0.20, P 〈 0.05 ). The PH was positively correlated with the rCBV in PCNSL ( r = 0. 82, P 〈 0.05 ). The PSR showed a significant negative correlation with rCBV in PCNSL( r = -0. 73 ,P 〈 0.05 ). The PH and PSR had no significantly correla- ted with the rCBV in HGG, respectively. Conclusion PWI plays an important role in differential diagnosis of PCNSL and HGG. PH and PSR of signal intensity-time curve had strong correlations with rCB~ in PCNSL, but those showed no correla- tion in HGG.
