摘要
目的探讨中药熏洗联合氟康唑治疗复发性外阴阴道假丝酵母菌病的疗效。方法将96例复发性外阴阴道假丝酵母菌病患者随机分为对照组和观察组,每组各48例,两组均给予氟康唑治疗,观察组加用中药熏洗外阴和冲洗阴道,共治疗6个月经周期;分析两组治疗效果,治疗前、治疗1、3和6个月的症状体征评分、阴道分泌物的带菌和炎症指标水平(白介素-6、肿瘤坏死因子-α)并记录治疗期间不良反应及停药6个月内的随访复发情况。结果观察组的总有效率、痊愈率分别为97.92%、29.17%,对照组分别为85.42%、10.42%,观察组均高于对照组(P<0.05);观察组治疗3、6个月的带菌率分别为54.17%、8.33%,均低于对照组的75.00%、22.92%(P<0.05),停药后6个月复发率观察组与对照组分别为18.75%、4.17%,观察组低于对照组(P<0.05);治疗后的症状体征评分、炎症指标水平均低于对照组,以上差异均有统计学意义;两组治疗期间的不良反应发生率差异无统计学意义。结论中药熏洗联合氟康唑治疗复发性外阴阴道假丝酵母菌病的效果较好,提高治疗有效率和痊愈率,降低带菌及炎症指标,并改善症状体征,安全性好且可耐受。
OBJECTIVE To explore the efficacy of Chinese herbal fumigation combined with fluconazole in the treatment of recurrent vulvovaginal cand^d~asis. METHODS Totally 96 patients with recurrent vulvovag[nal candidiasis were randomly divided into control group （n=48） and observation group （n~ 48）. Both groups were given fluconazole treatment for six menstrual cycles, while in addition only the observation group received Chinese herbal fumigation. The clinical effect, the signs and symptoms score, the contamination and the inflammatory markers of vaginal secretions before treatment and at 1st, 3rd and 6th months were compared between the two groups, and the adverse reactions during treatment and recurrence within six months of follow-up were recorded. RESULTS The total efficiency and cure rates of observation group were 97.92% ,29.17% and higher than 85.42% and 10.42o//00 of the control group （P^0.05） ~ the contaminated rate at 3rd and 6th months and the recurrence rate within six months of follow-up of observation group were 54.17% and 8.33% lower than 75.00% and 22.92% of the control group （P^0. 05）. The signs and symptoms score and levels of IL-6 and TNF-aof the observation group were 4. 17% lower than 8. 17% of the control group, with significant differences the difference in the incidence of adverse reactions during the treatment was not statistically significant. CONCLUSION The effect of Chinese herbal fumigation combined with fluconazole in the treatment of recurrent vulvovaginal candidiasis is better. It can increase the effective rate and the recovery rate, reduce the incidence of contamination and the level of inflammatory markers, improve the symptoms and signs, and has a good safety and tolerance.
