摘要
目的分析药房处方调剂中常见差错及其防范措施。方法分析2010年1月—2012年12月我院的药品调查调配差错登记记录,分析药房处方调剂中的常见差错,并给予相应的预防对策。结果 2010年1月—2012年12月我院共发生202例药房处方调剂差错,其中药物品种差错40例,药物数量差错56例,药物剂型与剂量差错22例,包装相似差错24例,划价差错12例,生产厂家差错22例,医师处方差错8例,退药差错10例,位置相近取药差错4例,发错科室4例。结论在药房处方调剂管理中建立健全调剂标准体系,加强药师培训、学习,建立调配工作的标准操作规程,完善考核制度等能够有效的提高药房药师的处方能力,有效预防药房处方调剂差错的发生。保证药房处方调配差错的减少,工作人员应端正工作态度,不断提高自身素质和能力。
Objective To analyze the common problems and prevention of dispense in pharmacy.Methods Choosed data of our pharmacy from Jan.2010 to Dec.2012 to analyze mistaken of dispense.Results There were 202 cases happened dispense mistake of dispense,including 40 cases of drug varieties errors,12 cases price error,56 cases error drug quantity,drug dosage form and dosage error 22 cases,8 cases physicians prescription errors,10 cases of medication errors,position error 4 cases similar to take medicine,4 cases were hair wrong department.Conclusion Establishing and perfecting the regulating standard in the management of prescription dispensing pharmacy system,Strengthen pharmacists training study,allocate work standard operating procedures,perfecting examination system can effectively improve the ability of pharmacy the pharmacist prescription,to prevent effectively happening of the pharmacy prescription dispensing errors.
