摘要
目的 探索高压氧（HBO）联合药物治疗梅尼埃病的中长期临床疗效.方法 将120名梅尼埃病（MD）患者按照治疗方法分为观察组（60例）和对照组（60例）.对照组给予常规药物治疗,观察组在对照组治疗的基础上给予HBO治疗.对HBO治疗前、治疗后1个月及6个月的治疗效果进行比较,并进行耳部检查和鼓室功能曲线测试,探讨HBO治疗的中长期疗效及并发症.结果 HBO组治疗后1个月及6个月DHI评分[分别为（39.22±14.47）分、（32.75±11.52）分]均显著低于治疗前[（58.18±20.35）分],差异均有统计学意义（P＜0.05）；HBO组听力和语言识别率提高,MD发作频率降低,与对照组相比差异有统计学意义（P＜0.05）；HBO组MD治疗显效率（68.33％）和有效率（93.33％）均高于对照组,差异均有统计学意义（P＜0.05）；2组不良反应均较少,差异无统计学意义（P＞0.05）.结论 应用HBO综合治疗MD疗效确切,治疗6个月临床疗效高于治疗1个月,且病情无反复.
Objective To investigate medium-long-term clinical effects of hyperbaric oxygen （HBO） combined with drugs on Ménière＇s disease MD.Methods In accordance with different treatment methods,120 patients with Ménière＇s disease were divided into the experimental group （n =60） and the control group （n =60）.Patients in the control group received routine drug treatment,while patients in the experimental group received HBO in addition to routine drug treatment,all with one treatment course of 10 sessions.Therapeutic effects were compared between pre-HBO therapy,one month after HBO therapy and 6 months after therapy.Aural test and tympanometry curve test were also performed to see the mid-tern effect of HBO,as well as complications that might occur.Results DHI scores 1 month and 6 months after therapy for the patients of theexperimental group were （39.22 ± 14.47） and （32.75 ± 11.52）,which were significantly lower than those before therapy,with statistical significance （P 〈 0.05）.Hearing and speech discrimination scores for the patients of the experimental group improved,and MD seizure frequency significantly decreased.Statistical significance could be seen,when comparisons were made between the 2 groups （P 〈 0.05）.MD control rate （68.33%） and effective rate （93.33 %） for the experimental group were all higher than those of the control group,with statistical significance （P 〈 0.05）.Few adverse reactions could be seen in the 2 groups,and no statistical significance could be noted in them （P 〉 0.05）.Conclusions HBO combined with routine drugs could produce reliable effect on Meniere ＇s disease,and 6-month comprehensive HBO therapy could produce better effect than that of 1 month comprehensive HBO therapy.What＇s more,no recurrence was seen and its therapeutic effect was definite.
