摘要
目的观察引流型喉罩用于头低脚高位腹腔镜手术时反流误吸的发生情况,评价其安全性。方法择期施行头低脚高位腹腔镜手术患者70例,年龄19～77岁,ASAⅠ或Ⅱ级,Mallam-patiⅠ～Ⅲ级。患者麻醉前20min口服自制胃溶性亚甲蓝胶囊。插入喉罩后即刻从胃管引流口和通气管口经纤支镜观察食管开口及声门开口处有无蓝染插的反流物,并分别在调整Trendelenburg体位(头低30°)后5、15min和手术结束时进行相同位置的观察,并在术后观察喉罩周边及内侧面是否蓝染。结果患者均顺利完成手术,其中有4例(5.7%)发生反流,无一例误吸。结论在择期头低脚高位腹腔镜手术中,正确应用引流型喉罩可有效防止术中反流误吸的发生。
Objective To evaluate the safety of proseal laryngeal mask airway on preventing reflux and aspiration for adult undergoing laparoscopic surgery in trendelenburg position. Methods Seventy adults （ASA I or II） aged 19-77 years and Mallampat{ I-m undergoing laparoseopic surgery in trendelenburg position were studied. Every patient took stomach soluble methylene blue capsule 20 min before anesthesia. Appropriate model of proseal laryngeal mask airway was chosen according to the patientr s weight. To detemaine whether was reflux dyed blue, the entrance to the esophagus and glottis was observed with bronchofiberscope through gastric tube and aeration tube immediately after laryngeal mask airway insertion, at 5 and 15 rains after adjusting the body position, and at the end of surgery. The surround and internal surface of proseal laryngeal mask airway was also checked after the surgery. Results The laparoscopic surgery was successfully performed in 70 cases, with reflux occurring in 4 cases （5.7%）. No aspiration was observed. Conclusion When used properly, proseal laryngeal mask airway can prevent reflux and aspiration for adult undergoing laparoscopic surgery in trendelenburg position~
