摘要
为总结左半结肠、直肠上段癌伴急性肠梗阻的外科治疗体会，回顾分析2005年1月至2012年11月于我科接受手术治疗的52例左半结肠及直肠上段癌伴急性肠梗阻患者资料，其中行结直肠癌一期切除吻合23例，一期切除吻合、回肠造口11例，扩大右半结肠切除、回肠与乙状结肠或直肠吻合15例，Hartmann手术3例。结果显示，结直肠癌一期切除吻合术后发生吻合口漏2例，再手术3例，死亡1例；一期切除吻合、回肠造口和扩大右半结肠切除回肠与乙状结肠或直肠吻合术后无吻合口漏发生，无再手术及死亡病例；Hartmann手术后无吻合口漏发生，再手术1例，无死亡病例。结果表明，左半结肠、直肠上段癌并发急性肠梗阻行一期切除吻合近端造口或扩大右半结肠切除一期吻合是较安全的术式。若行一期切除吻合应严格掌握适应症。
In order to summarize the surgical experience on acute intestinal obstruction associated with the carcinoma at left-colon or at upper segment of rectum,author retrospectively analyzed the data of 52 cases with this disease who were admitted from Jan. 2005 yr. to Nov. 2012 yr.. Among them,23 cases were sub- jected to primary resection and anastomosis of colorectal carcinoma,ll to primary resection and anastomo- sis as well ileostomy, 15 to extended resection of right-colon and ileum-sigmoid or ileum-rectum anastomo sis,3 to Hartmann procedure; As results, among the 23 cases 2 had anastomosis stoma leakage, 3 received secondary operation, 1 died;among the 11 cases and the 15 cases no anastomosis stoma leakage, secondary operation and death occured^among the 3 cases no anastomosis stoma leakage and death except for one re- ceived secondary operation. Results show that, for above-mentioned associated patients performing primary resectiong〉 anastomosis as well near end neostomy or extended resection of right-colon and ileum-sigmoid or ileum rectum anastomosis is safe, but it is needed strictly mastering the indication of primary resection . anastomosis.
