产前超声诊断胎儿临界值脑室扩张的临床意义 被引量:4

摘要 目的探讨产前超声诊断胎儿临界值脑室扩张的临床意义。方法选择妊娠20~24周行系统超声检查发现临界值脑室扩张(侧脑室宽度6~10 mm)胎儿286例,动态观察超声下胎儿侧脑室扩张妊娠中、晚期转归及合并其他畸形情况,随访妊娠结局及新生儿健康智力情况,并与轻度脑室扩张胎儿(侧脑室宽度11~15 mm)共98例及正常胎儿(超声检查5 mm以下)共384例进行比较。结果临界值脑室扩张组胎儿286例,合并畸形22例(7.7%),宫内好转125例(43.7%),引产15例(5.2%),生后发育异常新生儿14例(5.2%);轻度脑室扩张组胎儿98例,合并畸形20例(20.4%),宫内好转24例(24.5%),引产12例(12.2%),生后发育异常新生儿13例(15.1%);两组比较差异均有统计学意义(P<0.05)。临界值脑室扩张组胎儿与正常组胎儿引产率、出生后随访发现新生儿异常率及侧脑室宽度值宫内转归间差异均有统计学意义(P<0.05)。结论临界值脑室扩张胎儿的预后及生后新生儿健康率优于轻度脑室扩张胎儿,但相对于正常组胎儿仍然存在风险,临床应对此类胎儿引起重视。 Objective To investigate the clinical meaning of prenatal sonographic diagnosis of fetal borderline ventriculomegaly.Methods Women with 20-24 weeks of gestation were performed with systematic sonography and 286 cases of borderline ventriculomegaly(lateral ventricle width 6-10 mm) were detected.The progress of lateral ventriculomegaly at middle and late stage of pregnancy and other abnormalities were dynamically observed under ultrasound.Pregnancy outcome and newborn intelligence were followed up.The above parameters were compared between cases of borderline ventriculomegaly,mild ventriculomegaly(11-15 mm;n = 98),and normal fetuses( 5 mm;n = 384).Results Among the 286 cases of borderline ventriculomegaly,there were 22 cases(7.7%) of combined deformity,125(43.7%) intrauterine recovery,15(5.2%) induced labor,and 14(5.2%) neonatal abnormal development.Among the 98 cases of mild ventriculomegaly,there were 20 cases(20.4%) of combined deformity,24(24.5%) intrauterine recovery,12(12.2%) induced labor,and 13(15.1%) neonatal abnormal development.The abovementioned incidences were significantly different between the two groups(P 0.05).Significant differences were also observed between the fetuses with borderline ventriculomegaly and the normal fetuses in incidence of induced labor,incidence of postnatal abnormality,lateral ventricle width,and intrauterine recovery(P 0.05).Conclusion Fetuses with borderline ventriculomegaly have better prognosis and higher healthy neonate rate than those with mild ventriculomegaly,but still have higher risks compared with normal fetuses,therefore should be given enough attention in clinical care.
