冠状动脉综合征时血中C-反应蛋白、纤维蛋白原和白细胞的变化 被引量:41

摘要 目的 :探讨炎症反应和冠状动脉综合征 (CAS)之间的关系。方法 :测定 42例 CAS患者和 2 0例可疑冠心病 (KP)患者的血脂、C-反应蛋白 (CRP)、纤维蛋白原 (FIB)含量和白细胞 (WBC)总数 ,CAS患者经冠状动脉造影证实冠状动脉有异常性改变 ,KP患者经冠状动脉造影证实其冠状动脉正常。 CAS组患者又分为 3组 ,稳定型心绞痛 (SAP)组 17例 ,不稳定型心绞痛 (U AP)组 12例 ,急性心肌梗死 (AMI)组 13例。比较 CAS和 KP组之间 ,AMI、UAP、SAP与 KP组之间各项指标的差异。结果 :CAS和 KP组之间的血脂水平相比无显著性差异 ;CAS组的 CRP、FIB和 WBC水平明显高于 KP组 (P <0 .0 5 ) ;AMI、UAP组的 CRP、FIB和 WBC水平明显高于 KP组 (P值分别为 P <0 .0 1,P <0 .0 5 ) ;但 SAP和 KP组之间的 CRP、FIB和 WBC水平相比无显著性差异 (P >0 .0 5 )。结论 :CAS的发生与炎症反应有关 ,CRP、FIB和 WBC水平的增高与 CAS的发生显著相关。 Objective:To investigate the relationship between the inflammation and coronary artery syndromes (CAS).Method:Assessed the levels of serum lipids,C-reactive protein (CRP),fibrinogen (FIB) and white blood cell (WBC) in 42 patients with angiographically documented coronary artery disease (CAD) and 20 patients undergoing coronary angiography who had normal coronary angiograms.The patients with CAS were divided into three groups,13 cases of acute mycocardial infarction (AMI group),12 cases of unstable angina pectoris (UAP group),17 cases of stable angina pectoris (SAP group).Result:There was no significant difference in the serum lipids levels between CAS group and KP group.The levels of CRP,FIB and WBC in CAS group were higher than those in KP group (P< 0.01 ,P< 0.05 respectively).The levels of CRP,FIB and WBC in AMI group and UAP group were higher than those in KP group.But there was no significant difference in the levels of CRP,FIB and WBC between SAP group and KP group (P> 0.05 ).Conclusion:The results suggested that CAS was correlated with inflammaty response.The elevation of CRP,FIB and WBC has association with coronary artery syndromes.
