128层螺旋CT显示面神经管及其周围解剖结构在人工耳蜗植入术前中的应用研究

摘要 目的采用西门子128层螺旋CT利用多平面重组及曲面重建技术研究面神经管各段及周围解剖结构的关系,为临床人工耳蜗植入提供解剖依据。方法使用西门子definition AS+128层螺旋CT对100例(200侧耳)正常志愿者行颞骨0.6 mm薄层螺旋CT扫描,扫描后将原始数据传入西门子新沟通工作站进行图像后处理,通过面神经管的曲面重建图像,观察测量面神经管及其周围固定解剖位置与面神经管各段距离数值及测量面神经管各段数值。结果正常成年人不同性别组间面神经管迷路段长度及面神经管两个弯曲角度比较差异无统计学意义(P>0.05),正常成年人不同性别组间面神经管鼓室段长度及半规管隆突处与砧骨短脚距面神经管鼓室段最短距离比较差异无统计学意义(P>0.05),正常成年人不同性别组间面神经管乳突段长度、颈静脉窝及乙状窦距面神经管乳突段最短距离比较差异无统计学意义(P>0.05)。结论面神经管的128层螺旋CT 0.6 mm薄层扫描加多平面重组及曲面重建技术是研究面神经管CT解剖的良好影像学方法之一,能准确显示周围结构的毗邻关系,其结构对人工耳蜗植入及耳科疾病的影像诊断及手术治疗具有重要参考价值。 Objective To use 128-slice spiral CT and multi-planar reconstruction and surface reconstruction technique to investigate the correlation between facial canal in different segments and surrounding anatomic structure in order to provide anatomical basis for cochlear prosthesis implantation. Methods 100 healthy subjects (200 ears) were scanned by 128-slice spiral CT, including 108 females and 92 males (age range,20 ~ 70 yr) ,with an average of 43.23 + 14.49 scan undergoing facial canal surface reconstruction, facial canal and surrounding fixed anatomical location were observed and measured, and distance values of facial canal in different segments and the segment values of facial canal were calculated. Results There were no significant differences in the two bending angle of facial canal and the length of labyrinthine segment between males and females in adults (P 〉 0.05 ), there were no significant differences in the length of facial nerve tube tympanic segment and the shortest distance from semicircular canal protuberance as well as to tympanic segment of facial canal between males and females in adults ( P 〉 0.05 ), moreover, there were no significant differences in the length of mastoid segment of facial canal and the shortest distance from jugular fossa and sigmoid sinus to mastoid segment of facial canal between males and females in adults ( P 〉 0.05). Conclusion Facial canal high resolution spiral CT combined with muhiplanar reformation as well as surface reconstruction technique is one of the excellent iconography methods to study facial canal anatomic structure, which can accurately show adjacent relationship of surrounding structure of facial canal, which has important reference worth for cochlear prosthesis implantation and diagnosis for ear diseases.
