摘要
目的 探讨结直肠癌中胞外基质蛋白Periostin和基质金属蛋白酶-2(MMP-2)的表达及其与临床病理特征的关系和意义。方法 采用免疫组织化学EnVision二步法检测结直肠癌、管状腺瘤、绒毛状腺瘤及正常组织中Periostin和MMP-2的表达,分析各组间表达的差异及其与临床病理参数的关系。结果 在结直肠癌、管状腺瘤、绒毛状腺瘤和正常结直肠组织中Periostin的阳性率分别为83.7 %(41/49)、40.0 %(6/15)、32.1 %(9/28)、0(0/15);MMP-2的阳性率分别为71.4 %(35/49)、60.0 %(9/15)、64.3 %(18/28)、0(0/15),二者在各组间表达的差异有统计学意义(χ2=41.252,P=0.000;χ2=24.811,P=0.000)。Periostin阳性表达与患者性别(χ2=0.002,P=0.961)、年龄(χ2=2.267,P=0.132)、肿瘤部位(χ2=1.506,P=0.220)、分化程度(χ2=0.875,P=0.350)、淋巴结转移(χ2=3.315,P=0.069)均无关。MMP-2阳性表达与淋巴结转移有关(χ2=5.800,P=0.016),与患者性别(χ2=0.562,P=0.453)、年龄(χ2=0.138,P=0.711)、肿瘤部位(χ2=0.408,P=0.532)、分化程度(χ2=1.335,P=0.248)无关。Periostin和MMP-2表达在结直肠癌中呈正相关(r=0.332,P=0.020)。结论 Periostin和MMP-2的表达与结直肠癌的发生、发展密切相关,联合检测二者的表达能更准确地评估结直肠癌的生物学特征及预后。
Objective To investigate the clinicopathological significance of periostin and MMP-2 in colorectal carcinoma. Methods Immunohistochemistry was used to detect the expression of periostin and MMP-2 in colorectal carcinoma, tubular adenoma, villous adenoma and normal colorectal tissue. The correlations between the expression of periostin and MMP-2 with clinicopathologic parameters were analyzed. Results In colorectal carcinoma, tubular adenoma, villous adenoma and normal colorectal tissue, the positive rates of periostin were 83.7 % (41/49), 40.0 % (6/15), 32.1 % (9/28), 0 (0/15), respectively, while the positive rates of MMP-2 were 71.4 % (35/49), 60.0 % (9/15), 64.3 % (18/28), 0( 0/15), respectively. There were significant differences among the groups (χ 2 = 41.252, P = 0.000; χ 2 = 24.811, P = 0.000). The expression of periostin in colorectal carcinoma tissue were not correlated with sex (χ 2 = 0.002, P = 0.961), age (χ 2 = 2.267, P = 0.132), tumor sites (χ 2 = 1.506, P = 0.220), differentiation status (χ 2 = 0.875, P = 0.350) and lymphatic metastasis (χ 2 = 3.315, P = 0.069). The expression of MMP-2 in colorectal carcinoma were correlated with lymphatic metastasis (χ 2 = 5.800, P = 0.016), whereas not correlated with sex (χ 2 = 0.562, P = 0.453), age (χ 2 = 0.138, P = 0.711), tumor sites (χ 2 = 0.408, P = 0.532), differentiation degree (χ 2 = 1.335, P = 0.248). The expression of periostin and MMP-2 were positively correlated in colorectal carcinoma (r = 0.332, P = 0.020). Conclusion Periostin and MMP-2 are correlated closely with the development and progression of colorectal carcinoma. It might be helpful for evaluating the biological properties and prognosis of colorectal carcinoma, and it would be more accurate to use a combined analysis of the two indicators.
