摘要
目的探讨三维超声体积自动测量技术评估小儿肾积水患肾功能的价值。方法对62例单侧肾积水患儿分别行二维灰阶超声、三维超声及利尿性肾动态检查;采用二维超声测量患肾肾盂分离值和肾实质厚度,三维超声检查测量肾积水容积和患肾肾容积并计算患肾肾实质容积。对二维超声和三维超声所得数据与利尿性肾动态显像检查所得肾有效血浆流量进行相关性分析。结果二维所测肾盂分离为(30.2±14.4)mm,肾实质厚度为[7.1(4.2,9.6)]mm;三维所测肾积水容积、肾容积、肾实质容积分别为[28.0(9.1,77.4)]ml、[122.8(59.4,201.8)]ml、(87.6±46.9)ml。患肾有效血浆流量值显著低于健侧(67.1±44.4,84.2±48.9,P<0.001)。患肾肾实质容积值随肾有效血浆流量值下降而逐渐减小,两者呈正相关(r=0.585,P<0.001);而肾盂分离、肾实质厚度、肾积水容积、肾容积均与肾有效血浆流量无明显相关性(P>0.05)。结论三维超声体积自动测量技术所测肾实质容积值可以间接反映肾积水患儿患肾功能情况,有助于制定小儿肾积水治疗方案和预后评估。
Objective To investigate the value" in three-dimensional ultrasound virtual organ computer-aided analysis (VOCAL) technique in invalidating the function of diseased kidney in children with hydronephrosis. Methods A total of 62 children with unilateral hydronephrosis were enrolled. All of them firstly underwent conventional ultrasound examina- tion to determine the pelvicaliceal split and parenchymal thickness, and then underwent three-dimensional ultrasound exami- nation to obtain pelvicaliceal volume (PCV), renal volume (RV) and renal parenchymal volume (RPV). Finally the diuretic renography examination was performed to determine effective renal plasma flow (ERPF). Correlation analysis was per- formed between the ultrasound results and ERPF values. Results The pelvicalicea[ split, parenchymal thickness, PCV, RVandRPVvalue of the diseased renal was (30.2± 14.4)mm, 7.1 (4.2,9.6)]mm, [28.0 (9.1,77.4)]ml, [122.8 (59.4,201.8) ]ml and (87.6 ± 46.9)ml, respectively. The mean ERPF value of the diseased kidney was significantly lower than that of the normal kidney (67.1±44.4, 84.2±48.9, P〈0. 001). RPV of the diseased kidney positively correlated with ERPF (r=0. 585, P〈0. 001). There was no significant correlation between ERPF and pelvicaliceal split, parenchy- real thickness, PCV nor RV (all P〉0.05). Conclusion RPV value provided by three-dimensional ultrasound with VO- CAL technique can provide valuable prediction of the function of diseased kidney in children with hydronephrosis, which is helpful to the treatment planning and prognosis assessment.
