头颈部弥漫大B细胞淋巴瘤的免疫组织化学分型分析

Analysis of the Immunohistochemical Subtypes of Diffuse Large B-cell Lymphoma of Head and Neck Region

摘要 目的:检测头颈部弥漫大B细胞淋巴瘤的免疫组织化学分型,并对Hans、Choi和Tally三种分型法进行比较,同时观察各蛋白的表达在各种分型法的意义。方法:收集我院2010年5月到2012年10月共103例头颈部弥漫大B细胞淋巴瘤,进行CD10、Bcl-6、MUM1、GCET1、FOXP1和LMO_2蛋白表达的免疫组织化学检测,并进行Hans、Choi和Tally三种分型。结果:103例病例中,CD10、Bcl-6、MUMl、GCET1、FOXPl和LMO_2的阳性例数分别为16例(15.53%)、68例(66.02%)、69例(66.99%)、21例(20.39%)、75例(72.82%)、50例(48.54%);Hans分型中,生发中心B细胞(GCB)型26例(25.0%),活化B细胞(ABC)型77例(75.0%);Choi分型中,GCB型25例(24.0%),ABC型78例(76.0%);Tally分型中,GCB型20例(19.4%),ABC型83例(80.6%)。CD10、MUM1、GCET1、FOXP1和LMO_2表达情况对于分型诊断结果均具有统计学意义(P<0.05),而Bcl-6的表达对于Hans和Choi分型结果无统计学意义。结论:头颈部弥漫大B细胞淋巴瘤以ABC型多见;Hans、Choi和Tally三种分型法之间无明显差别;CDl0、MUM1、GCET1、FOXP1和LMO_2的表达情况对于诊断结果均具有意义。 Objective: To detect the immunohistochemical subtypes of diffuse large B-cell lymphoma of head and neck region, compare the Hans, Choi and Tally algorithm and observe the significances of proteins expression in the three algorithms. Methods : One hundred and three cases of DLBCL of Sichuan Cancer Hospital from May 2010 to October 2012 were studied using immunohistochemis- try method for CDIO, Bcl-6, MUM1, GCET1, FOXP1 and LMO2. Hans, Choi and Tally subtypes were classified. Results: The re- suhs showed that 16 cases ( 15.53 % ) were positive for CD10, 68 cases ( 66. 02% ) for Bcl- 6, 69 cases ( 66. 99% ) for MUM1,2l cases (20. 39% )for GCET1,75cases(72. 82% )for FOXP1, 50 cases(48. 54% )for LMO2. In Hans classification, 26 cases (25.0%) were GCB and 77cases(75.0% ) were ABC subtype. In Choi classification, 25 cases (24. 0% )were GCB and 78 cases(76. 0% ) were ABC subtype. In Tally classification, 20 cases ( 19.4% ) were GCB and 83 ( 80. 6% ) cases were ABC subtype. The expression of CD10, MUM1,GCET1,FOXP1 and LMO2 cotrelated with the classification results(P 〈 O. 05 ) ,but the expression of Bcl-6 did not significantly correlate with Hans and Choi classifications. Conclusion: The ABC type is more commonly found than GCB in diffuse large B-cell lym- phoma of head and neck region. There is no significant difference among Hans, Choi and Tally classification. The expressions of CD10, MUM1, GCET1, FOXPI and LMO2 correlate with the classification results.
