摘要
目的研究分析胃癌手术后感染发生的危险因素。方法回顾性分析2005年2月至2012年2月间收治的400例胃癌手术后患者,统计分析患者术后感染发生率与相关因素的相关性。结果胃癌术后感染率与年龄大小、吸烟年支多少、手术范围、术前预防性抗生素应用、术前输血量、白细胞水平、Th细胞水平、病理分期和住院时间有关,另外BMI过小者比正常和偏胖者感染率更高,差异有统计学意义(P<0.05);随着年龄的增长、吸烟年支的增加、手术范围的扩大、输血量的增加、Th细胞水平的降低和病理分期的延后,胃癌患者术后感染率上升;年龄、吸烟、手术范围、输血量和病理分期是影响术后感染率的独立危险因素,其中术前输血量的相对危险度最高(P<0.05)。结论可以从年龄大小、吸烟年支多少、手术范围、术前预防性抗生素应用、术前输血量、白细胞水平、Th细胞水平、病理分期和住院时间多方面考虑降低胃癌患者术后的感染率。
Objective To study the risk factors for gastric cancer postoperative infection occurred.Methods A retrospective analysis of 400 cases of postoperative gastric cancer patients admitted to our hospital from February 2005 to February 2012,the statistical analysis of patients infected with the correlation of incidence and related factors.Results Gastric postoperative infection rates with age,smoking years support related to the number and extent of surgery,preoperative prophylactic antibiotics,preoperative transfusion,white blood cells,Th cell level,pathologic stage and duration of hospitalization,the other BMI is too small infection rate higher than normal and too fat by the difference was statistically significant（P 0.05）;increase with age,the increase in smoking years support the expansion of the extent of surgery,transfusion increase in the amount of reduction in Th cell level pathological stage delayed increase in postoperative infection rates in patients with gastric cancer;,age,smoking,extent of surgery,blood transfusion and pathological staging is an independent risk factor for postoperative infection rates,preoperative blood transfusion relative risk for the highest（P 0.05）.Conclusions from age,smoking years support the number and extent of surgery,preoperative prophylactic antibiotics,preoperative transfusion,the level of white blood cells,Th cell level,pathologic stage,and hospitalization time reduce the infection of the gastric cancer patients in many ways rate.
