摘要 目的了解某煤业集团煤工尘肺患者发病特点,为预防和控制煤工尘肺病提供科学依据。方法对某煤业集团公司从1963-2012年经重庆市尘肺病诊断机构诊断的300名尘肺病患者,分析不同诊断年代和工种在潜伏期、接尘年龄、诊断年龄及晋期时间上的变化趋势。结果某煤业集团公司煤工尘肺患者300人,其中1期119人,2期129人,3期52人;平均晋期时间为9.7年,平均潜伏期为27.4年,平均接尘年龄为26.5岁,平均诊断年龄为53.9岁,工种主要以采煤和掘进为主。研究发现随着诊断年代的推移,诊断年龄、潜伏期均呈明显增长趋势,各组之间比较差异有统计学意义(P<0.01)。不同工种间的平均诊断年龄、平均潜伏期、平均接尘年龄差异均有统计学意义(P<0.01);总的平均晋期时间为9.7年,1期晋升为2期的患者129人,平均晋期时间为9.2年,2期晋升为3期的患者52人,平均晋期时间为10.8年。结论该煤业集团接尘工人的潜伏期和诊断年龄均有延长的趋势。 Objective To provide scientific evidences for preventing and controlling pneumoconiosis by investigating coal workers suffering from pneumoconiosis disease in a coal industry group. Methods Coalworkers in the period of 1963 - 2012 in a coal industry group diagnosed with pneumoconiosis in Chongqing were analyzed in terms of dynamic trends of different diagnostic age and types of work with aspects of dust working age, dust - contacting age, diagnosis age, incuba- tion period and deterioration period. Results Of the total 300 pneumoconiosis workers, 119 people were of pneumoconio- sis stage I , 129 of pneumoconiosis stage I1 ,and 52 of pneumoconiosis stage III. The average deterioration period was 9.7 years, the average incubation period of dust exposure was 27.4 years, the average dust exposure age was 26.5 years and the mean age of diagnosis was 53.9 years old. The main types of work in coal were mining and tunneling. With the growth of diagnostic age, age of diagnosis, dust exposure and latency were all showed increasing tendency, and the difference between groups were all statistically significant( all P 〈 0. 01 ). The mean age of diagnosis, mean incubation period and the average dust exposure age were all statistically different on different type of work (all P 〈 0. 01 ). The av- erage time of deterioration period was 9. 7 years, 129 patients of pneumoconiosis stage I were promoted to stage II, with an average time of 9. 2 years;52 patients of stage II were pro years.
