摘要
目的探讨3.0TMR扩散加权成像(DWI)及表观扩散系数(ADC)对前列腺癌的诊断价值。资料与方法经超声引导穿刺或病理证实的88例前列腺癌及20例良性前列腺增生(BPH)患者于术前行3.0TMRDWI检查,b值取0、100、800s/mm2。所有患者取癌灶、BPH结节、正常前列腺周围带(PZ),并比较其ADC值,计算前列腺癌在DWI(b=800s/mm2)的阳性显示率。采用受试者工作特征曲线(ROC曲线)及配对χ2检验比较ADC值及DWI(b=800s/mm2)对前列腺癌的诊断效能。结果三种组织间ADC值存在重叠但差异显著(F=937.734,P<0.001),DWI(b=800s/mm2)的前列腺癌阳性检出率为82.4%。ROC曲线分析显示,当ADC值为1.23×10-3mm2/s时,曲线下面积为0.999,诊断前列腺癌的敏感度及特异度均为99.2%;当b=800s/mm2时对前列腺癌的检出率为82.4%(98/119),曲线下面积为0.912,两种方法对前列腺癌的诊断效能差异有统计学意义(F=937.734,P<0.001)。结论 ADC值及DWI对前列腺良恶性组织的鉴别有较大价值,且ADC值优于DWI。
Purpose To evaluate the value of 3.0T MR diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC) in the diagnosis of prostatic carcinoma. Materials and Methods Eighty-eight patients with prostatic carcinoma and 20 patients with benign prostatic hyperplasia (BPH) who were diagnosed pathologically by surgical or ultrasound guided puncture biopsy underwent 3.0T MR DW1 before operation (b=0, 100, 800 s/ram2). The ADC values of prostate carcinoma or BPH nodules, and peripheral zone (PZ) of all patients were measured and the positive rate of prostate carcinoma in DW1 (b=800 s/mm:) was also calculated. The diagnostic effect of ADC value and DWI (b-800 s/mm-) was compared using receiver operating characteristic curve (ROC curve) and paired y- test. Results The ADC values of the three kinds of tissue were overlapping but the difference was significant (F=937.734, P〈0.001); the positive rate of prostate carcinoma in DWI (b=800 s/mm2) was 82.4%. The ROC curve demonstrated significant diagnostic effect of both ADC value and DWI (b=800 s/mm2) in prostate carcinoma (area under the curve: 0.999 and 0.912, respectively), with a significant difference (F=937.734, P〈0.001 ). Conclusion Both ADC value and DWI have great value in the differential diagnosis of benign and malignant prostatic tissue, while the former is better.
