摘要
目的:探讨窒息早产儿脑血流动力学变化及其与早产儿脑损伤的关系。方法:对44例窒息早产儿在生后24 h内应用彩色多普勒超声测定大脑中动脉脑血流速度及阻力指数,并与30例无窒息的正常早产儿进行比较分析;所有病例定期行头颅超声检查,比较两组早产儿脑损伤的发生情况。结果:重度窒息组脑血流速度显著低于轻度窒息组和对照组,差异有统计学意义(Pa<0.01),轻度窒息组显著低于对照组,差异有统计学意义(P<0.01),窒息程度越重,脑血流速度越慢;重度窒息组阻力指数(RI)显著高于轻度窒息组和对照组,差异有统计学意义(Pa<0.01),轻度窒息组RI显著高于对照组,差异有统计学意义(P<0.01)。窒息组早产儿脑损伤发生率显著高于对照组,差异有统计学意义(P<0.05)。脑损伤组脑血流速度明显低于无脑损伤组,差异有统计学意义(P<0.05)。结论:早产儿窒息后出现脑血流动力学改变,可使早产儿脑损伤的发生率增加。应用彩色多普勒超声检测窒息早产儿脑血流动力学变化,对判断病情程度及预后评估、指导治疗有重要价值。
Objective:To investigate the cerebral hemodynamic changes in asphyxiated preterm infants and its relationship with brain injury.Methods:Color Doppler ultrasonography was used to monitor the changes of cerebral blood flow of 44 asphyxiated preterm infants in 24 h after birth,then the results were compared with 30 healthy control preterm infants at the same period.Then all the infants were examined by brain ultrasonography periodically.The differences of incidence of brain injury between the two groups were evaluated.Results:Cerebral blood flow velocities in severe asphyxia group were decreased significantly compared with those of the mild asphyxia group and the control group(Pa<0.01).The resistance index(RI) in severe asphyxia group was increased significantly compared with that of the mild asphyxia group and the control group(Pa<0.01).The incidence of brain injury was increased significantly in asphyxia group than that of the control group(P<0.05).Cerebral blood flow velocities in brain injury group were decreased significantly compared with those of no brain injury group(P<0.05).Conclusion:Owing the cerebral hemodynamic changes in asphyxiated preterm infants,the incidence of brain injury in preterm infants are increased.Observing cerebral hemodynamic changes in asphyxiated preterm infants is helpful to evaluate the extent of brain injury as well as to instruct therapy.
