摘要
目的观察养正消积胶囊联合化疗治疗中晚期大肠癌的临床疗效和安全性。方法将320例中晚期大肠癌患者随机分为治疗组和对照组,各160例。对照组单用FOLFOX方案治疗,治疗组予养正消积胶囊联合FOLFOX方案治疗,14天为1个周期,共治疗4个周期。观察实体瘤客观疗效、中医临床证候疗效、治疗前后生活质量(KPS)评分及NK细胞、T细胞亚群、VEGF和IL-8水平变化及不良反应情况。结果治疗组和对照组实体瘤客观疗效总有效率分别为38.13%和13.13%,临床受益率分别为82.5%和48.13%;治疗组实体瘤疗效优于对照组(P<0.05)。治疗组和对照组中医临床证候改善率分别为83.13%和44.38%;治疗组中医临床证候疗效优于对照组(P<0.05)。治疗组生活质量(KPS评分)改善优于对照组(P<0.05)。治疗组治疗后免疫指标、VEGF和IL-8水平的变化均较治疗前有显著性差异(P<0.05),且与对照组治疗后比较有显著性差异(P<0.05)。治疗组在骨髓抑制等方面的毒副反应少于对照组(P<0.05)。结论养正消积胶囊联合化疗治疗中晚期大肠癌可缩小病灶,改善临床证候,提高患者生活质量和免疫功能,下调VEGF和IL-8水平,减轻化疗毒副反应。
Objective To observe the clinical efficacy and safety of ＂Yangzheng Xiaoji Capsule＂ combining with chemotherapy in treating advanced colon and rectal cancer. Methods Three hundred and twenty cases were randomized into treatment group and control group, 160 cases in each group. The control group was treated with FOLFOX, and treatment group was added with ＂Yangzheng Xiaoji Capsule＂, with 4 courses and 14 days is a course. The curative effect of solid tumor and TCM clinical symptoms, and KPS, NK cell, T-lymphocyte subsets, VEGF, IL-8 level and adverse effect were observed. Results The overall effective rate was 38.13% in the treatment group and 13.13% in the control group, and the clinical benefit rate was 82.5% in the treatment group and 48.13% in the control group ; the curative effect of solid tumor in treatment group was better than the controls （ P 〈 0.05 ）. The improvement rate of TCM clinical symptoms was 83.13% in treatment group and 44.38% in the control group, with significant difference between the two groups （ P 〈 0.05 ）. The improvement of living quality （ KPS score） in treatment group was superior to the controls （ P 〈 0.05 ）. There were significant differences in immunity index, VEGF and IL-8 level between before and after treatment in treatment group （ P 〈 0.05 ） , and with significant difference between treatment group and control group （ P 〈 0.05 ）. The toxic and side effect on myelosuppression in treatment group was less than the controls （ P 〈 0.05 ）. Conclusion ＂Yangzheng Xiaoji Capsule＂ can reduce the focus, improve the clinical symptoms, improve life quality and immunologic function, reduce VEGF and IL-8 level, alleviate toxic and side effect of chemotherapy in treating advanced colon and rectal cancer.
