摘要
目的探讨联合检测尿肾损伤分子-1(Kim-1)和血超氧化物歧化酶(SOD)对糖尿病肾病(DN)早期诊断的价值。方法检测30例早期DN患者、30例单纯糖尿病(DM)患者及30例正常成人的尿Kim-1、尿白蛋白排泄率(UAER)和血SOD、肌酐(Scr)、尿素氮(BUN)并进行对比分析。结果早期DN组、单纯DM组的尿Kim-1和血SOD水平均高于对照组,差异有统计学意义(p<0.05);3组患者的血Scr及BUN水平间差异无统计学意义(p>0.05);早期DN组患者的尿Kim-1与UAER呈正相关(r=0.974,p<0.05),SOD与UAER呈负相关(r=-0.948,p<0.05)。结论 Kim-1可能通过氧化应激途径参与DN的发展,联合检测尿Kim-1和血SOD在DN患者肾损害早期诊断中具有一定的临床价值。
Objective To probe into the value of combined detection of urinary kidney injury molecule-1(Kim-1) and blood superoxide dismutase(SOD)in early diagnosis of diabetic nephropathy(DN).Method A detection of urinary Kim-1,urinary albumin excretion rate(UAER),blood SOD,serum creatinine(Scr)and blood urea nitrogen(BUN)in 30cases of patients with early DN,30cases of patients with non-proliferative diabetes mellitus(DM)and 30normal adults was conducted and the results were comparatively analyzed.Results Level of urinary Kim-1and blood SOD in the group with early DN and that with non-proliferative DM was higher than the control group,and the differences were statistically significant(p0.05);there was no statistically significant difference(p0.05)in the level of blood Scr and BUN among the three groups;urinary Kim-1of patients in the group with early DN had positive correlations with UAER(r=0.974,p0.05),and SOD had negative correlations with UAER(r=-0.948,p0.05).Conclusion Kim-1may be involved in the development of DN by means of oxidative stress,and combined detection of urinary Kim-1and blood SOD has certain clinical values in early diagnosis of renal damage in patients with DN.
