七氟烷、异氟烷分别联合丙泊酚用于腹部手术麻醉中的体会 被引量:12

Effect of sevoflurane and isoflurane combined with propofol in the anesthesia of abdominal surgery

摘要 目的 观察七氟烷、异氟烷吸入分别联合丙泊酚用于腹部手术的麻醉效果.方法 将90例ASAⅠ~Ⅱ的患者按麻醉方法分为A组(n=30,丙泊酚)、B组(n=30,异氟烷吸入联合丙泊酚)及C组(n=30,七氟烷吸入联合丙泊酚).术中监测各时点HR、MAP,术后随访患者主观感受及其他各项指标.结果 3组患者麻醉效果满意,C组患者术毕拔管时间及术毕完全清醒时间均明显低于A、B组(P<0.05);A组患者术毕烦躁率明显高于B、C组(P<0.05);A组丙泊酚的总用量明显高于B、C组,C组拔管后疼痛率、术中知晓率明显低于A、B组(P<0.05).结论 七氟烷吸入联合丙泊酚麻醉术后效果明显优于异氟烷吸入联合丙泊酚静脉麻醉以及单纯丙泊酚静脉麻醉. Objective To observe anesthetic effects of sevoflurane inhalation combined with propofol and isoflurane inhalation combined with propofol in the anesthesia of abdominal surgery and compare the related index.Methods Ninety abdominal surgery patients with ASA level Ⅰ-Ⅱ were randomly divided into propofol group (group A,n =30),isoflurane combined with propofol group(group B,n =30)and sevoflurane combined with propofol group(group C,n=30).HR and MAP were monitored in different time points of intraoperative period.Subjective feelings and other indexes were observed by follow-up method in postoperative period.Results All of patients had satisfactory effects of aesthesia.Postoperative extubation time and postoperative awakening time in group C were significantly shorter than group A and B (P < 0.05),postoperative irritation rate in group A was significantly higher than group B and C (P < 0.05).Total quantity of propofol in group A was significantly higher than group B and C,pain rate after extubation and intraoperative awareness rate in group C were significantly lower than group A and B (P < 0.05).Conclusion Anesthetic effect of sevoflurane inhalation combined with propofol is better than isoflurane inhalation combined with propofol and isolated use of propofol.
