摘要
目的探讨不同降压药物口服超前控制性降压在鼻腔内窥镜手术中临床效果。方法将110例ASA 1~2级行择期手术患者随机分成2组,每组55例。观察组口服硝苯地平缓释片行超前控制性降压;对照组使用硝酸甘油行术中控制性降压。对比2组患者麻醉前、插管后、术中、拔管时、拔管后30 min、回病房后3h的平均动脉压（MAP）、心率（HR）,并分别记录术中补液量、术中出血量和术后创面渗血量等情况。结果 2组患者术中血流动力学稳定,术中出血量比较差异无统计学意义（P〉0.05）,但观察组术后渗血量显著少于对照组（P〈0.05）;观察组患者MAP较对照组患者更为稳定,组间比较差异显著（P〈0.05）。结论口服硝苯地平缓释片行超前控制性降压下进行鼻内窥镜手术,能保证术中血压平稳、术中血压控制理想;术前术中血流动力学稳定、术后渗血量少,是鼻内窥镜手术控制血压的有效药物。
Objective To study the controlled hypotension effect of different antihypertension drugs for nasal endoscopic surgery on early stage.Methods A total of 110 ASA Ⅰ-Ⅱ patients for selective operation were randomly divided into trial group and control group,with 55 in each.To control hypertension in surgery,the trial group was treated with nifedipine delayde-release tablets（p.o.） before operation,while the control group was treated with nitroglycerin during operation.Two groups′ hemodynamics indexes were recorded,including the intraoperative rehydration fluids,blood loss,postoperative wound ooze blood,and mean arterial pressure（MAP） and heart rate at the time of preanesthesia,postintubation,intraoperation,extubation,30 min after extubation,3 h after being returned to the ward.Results The intraoperative hemodynamics indexes kept stable.There was no significant difference in the intraoperative blood loss between the two groups（P〈0.05）,while the control group showed less postoperative wound ooze blood and more stable MAP（P〈0.05）.Results Using nifedipine delayde-release tablet before operation is effective to control blood pressure during nasal endoscopic surgery,with stablizing blood pressure before and during surgery,and reducing wound ooze blood after surgery.
