摘要
目的通过对上前牙全瓷桥的抗折性进行正交试验研究,探讨上前牙全瓷桥的合理设计方案。方法根据正交表L16(42×29)对全瓷材料、连接体宽度、粘固材料与底层冠舌侧设计四个因素进行搭配制作16个三单位上前牙全瓷桥,在电子万能测试仪上进行断裂试验,对其断裂力值进行极差分析及正交试验的方差分析。结果全瓷材料对断裂力值的影响最大,连接体宽度与底层冠舌侧设计其次,粘固材料影响最小。四种全瓷材料的抗折强度由大到小依次为:氧化钇部分稳定型氧化锆(YZ)、纯氧化铝(AL)、玻璃渗透氧化铝(ICA)、易美压铸陶瓷(E.max)。ICA、AL、YZ三种材料上前牙全瓷桥一侧连接体断裂,E.max全瓷桥两侧连接体均断裂。选用YZ制作三单位固定桥,而且桥体两侧的连接体宽度均为3.0mm,底层冠舌侧厚度为1.0mm时,全瓷桥的断裂力值最大。结论 ICA、AL、YZ均可满足前牙区正常咀嚼力的需要,临床根据实际情况来选择制作全瓷桥。为增强上前牙三单位全瓷桥的抗折性,推荐全瓷桥双侧连接体面积均不小于4.0mm×3.0mm;双侧基牙的底层冠舌侧厚度达到1.0mm。
Objective To provide guidance for relatively reasonable design of all-ceramic fixed partial dentures (FPDs) by the orthogonal experimental study of fracture resistance of maxillary anterior applications. Methods Four experimental factors, including ceramic materials, connector width, lingual thickness of core and adhesive agents were combined according to the orthogonal table L16(4^2×2^9). Ceramic materials tested in this study are Vita In-Ceram Alumina (ICA), Vita In-Ceram AL (AL), Vita In-Ceram YZ (YZ) and IPS E.max press (E.max). On the basis of the orthogonal table, 16 three- unite all-ceramic FPDs were fabricated, and then underwent load until fracture. Fracture forces were statistically analyzed. Results Ceramic materials had greater influence on the fracture forces, followed by the connector width and lingual thickness of core, adhesive agents had the least influence. Fracture strength of four all-ceramic materials descends by YZ 〉 AL 〉 ICA 〉 E.max. All the ceramic FPDs made of ICA, AL, YZ fracture at unilateral connector, and E.max all-ceramic FPDs fracture completely at bilateral connectors. The YZ all-ceramic FPDs with 3.0 mm of bilateral connector width show the largest load, while their lingual thickness of core is 1.0 mm. Conclusions ICA, AL and YZ can meet the mastication in anterior locations, and it can be choose clinically according to the actual situation. The recommended minimum connector area of bilateral connectors is 4.0 mm×3.0 mm or more. The fracture resistance of an anterior 3-unit all-ceramic FPD can be improved when the lingual space is filled with core materials.
