摘要
目的 探讨下腹部横行腹直肌皮瓣用于皮肤软组织缺损的修复及乳房再造。方法 以双侧腹壁下动脉为血管蒂的下腹部横行腹直肌皮瓣进行吻合血管游离移植。结果 临床应用 10例 ,4例用于烧伤瘢痕挛缩畸形的整形治疗 ,3例用于创伤一期修复 ,3例用于乳腺癌根治切除术后乳房再造。皮瓣面积为 10 35cm2 至 12 40cm2 ,全部成活 ,经 3mon至 5a随访效果良好 ,未见并发症发生。结论 该皮瓣面积大 ,可用于大面积的皮肤软组织缺损的修复及乳房再造等 ,皮瓣切口相对隐蔽 ,供区一般均可直接缝合 ,切口疤痕隐蔽理想 ,术后供区无明显功能影响 ,并且还常常收到腹壁整形美容的效果。腹壁下动脉血管恒定 ,口径较大 ,蒂长 ,吻合方便。
AimTo expiore the use of free Transverse Rectus Abdominis Myocutaneous(TRAM)flap.Methods TRAM flaps of double inferior abdominis vessels were free transferred by vascular anastomosis.Results This method has been used on 10 cases for wound repair(3 cases),breast reconstruction(3 cases),and burn scar resection(4 cases).The flap size is 10 35cm2 to 12 40cm2 and all flaps survived well.They showed no complications in a following up of 3 months to 5 years.Conclusion This method is dependable from the view of flap blood perfusion and it is acceptable for the donor site scar.It is especially wonderful for large defect repair and breast reconstruction.
