地佐辛、芬太尼、舒芬太尼复合罗哌卡因增快麻醉起效时间及阵痛疗效的观察 被引量:24

摘要 目的:比较等效剂量的地佐辛、芬太尼、舒芬太尼复合甲磺酸罗哌卡因用于增快硬膜外麻醉起效时间及增强阵痛疗效的临床观察。方法:选择硬膜外麻醉手术90例,随机分为A组(地佐辛组)、B组(芬太尼组)和C组(舒芬太尼组),每组各30例。3组年龄、性别、体质量、手术时间等比较差异无统计学意义,具有可比性。以VAS评分、BCS评分方法评价硬膜外麻醉起效时间一镇痛效果,并观察患者是否出现嗜睡、头晕、恶心呕吐、寒战、呼吸抑制以及皮肤瘙痒等不良反应。结果:3组患者硬膜外麻醉起效时间B组高于A组和C组(P<0.05);3组患者在硬膜外麻醉后5min(T0),10min(T1),15min(T2),20min(T3)时点VAS评分A组和C组低于B组(P<0.05),BCS评分A组和C组高于B组(P<0.05);3组间下肢运动阻滞评分比较无显著差异(P>0.05);C组嗜睡、头晕、恶心呕吐、呼吸抑制、皮肤瘙痒等不良反应高于A组和B组(P<0.05);A组和B组之间差异无显著性(P>0.05)。结论:地佐辛复合甲磺酸罗哌卡因用于硬膜外麻醉起效快、镇痛效果确切、不良反应轻、安全可行。 OBJECTIVE To compare and observe the use of equivalent doses of dezocine,fentanyl and sufentanil compounded by ropivacaine mesylate to speed up the onset time of epidural anesthesia and increase the analgestic curative effect.METHODS 90 cases of patients with CEA operations were divided at random into Group A(dezocine group),Group B(fentanyl group) and Group C(sufentanil group),30 cases for each group.There was no significant difference in the age,gender,body weight and operation time between the three groups.The onset time of CEA operation and the analgestic effect were assessed with the methods of VAS and BCS scores and the patients were observed to see whether they had adverse reactions such as feeling dizzy,nausea,vomiting,shivering with cold,respiratory inhibition as well as skin itch,etc.RESULTS The results showed that in the three groups of patients with epidural anesthesia,the onset time of B group was higher than that in A group and C group(P〈0.05);among the three groups of patients,at the 5 min(T0),10 min(T1),15 min(T2),20 min(T3)points of time upon the completion of CEA,for the VAS score,Groups A and C were lower than Group B(P〈0.05);for the BCS score,Groups A and C were higher than Group B(P〈0.05);for the comparison of lower limb motor stop marking,there was no obvious difference among the three groups(P〈0.05);and for the incidence of adverse reactions such as feeling dizzy,nausea,vomiting,respiratory inhibition,skin itch etc,Group C was higher than Groups A and B(P〈0.05),but there was no difference between Groups A and B(P〈0.05).CONCLUSION The use of dezocine compounded by ropivacaine mesylate for CEA operation,,the onset of effect was quickly,had good analgestic effect and less adverse reactions.So it is safe and feasible.
