摘要 目的:探讨GnRH拮抗剂应用于FSH促排卵人工授精周期中的作用及临床价值。方法:全面检索数据库,获得相关临床随机对照(RCT)文献9篇,共纳入患者1 771名,其中拮抗剂组858名,对照组913名,对其进行质量评价、筛选和异质性检验,应用Rev Man5.0软件对各研究的原始结果进行统计处理。结果:拮抗剂组与对照组相比,周期妊娠率、多胎率、早期流产率、周期取消率无明显差异,卵泡过早黄素化显著降低(17.2%vs 3.6%),差异具有统计学意义(P<0.05)。结论:在FSH促排卵的IUI中加用GnRH拮抗剂能有效地抑制内源性LH水平,显著降低卵泡黄素化,适用于既往周期有明确黄素化史的患者。 Objective:To assess the effects and clinical value of GnRH antagonist in FSH stimulated intrauterine insemination(IUI)cycles.Methods:Databases including the Medline,EMBASE,Cochrane library,Clinical trial were searched for prospective trials and 9random control trials(RCT)were reviewed in this meta-analysis by Rev Man 5.0soft ware.858patients who were administered GnRH antagonist and 913conservatively treated patients who served as control subjects were included.Result:Premature luteinzation was significant lower in GnRH antagonist cycles compared to the control cycles(17.2%vs.3.6%,P0.05),while there were no statistical differences in pregnancy rate(PR),multi pregnancy rate,miscarry rate,and cycle cancel rate between two groups in evaluation criteria.Conclusion:GnRH antagonist in FSH/IUI cycles may be beneficial by decreasing premature luteinzation rate safety and efficiently,which may be an alternative treatment option for subjects with previous proven luteinization.
