胃癌患者hMLH1基因启动子区CpG岛甲基化状态及其与蛋白表达的关系

摘要 目的:探讨胃癌组织中hMLH1基因启动子区CpG岛甲基化及蛋白的表达与胃癌生物学行为的关系,并分析hMLH1基因启动子区CpG岛甲基化与其蛋白表达的相关性。方法:采用甲基化特异性PCR(methylation-specific PCR,MSP)技术和免疫组织化学SP法检测45例患者胃癌组织、正常胃组织中hMLH1基因启动子区CpG岛甲基化情况及蛋白的表达。结果:hMLH1基因启动子区CpG岛在胃癌及正常组织中的甲基化阳性率分别为33.3%和0(P<0.05);hMLH1蛋白在胃癌及正常组织中的阳性率分别为57.8%和95.2%(P<0.05);hMLH1基因启动子区CpG岛甲基化及蛋白的表达与胃癌的临床病理参数无关(P>0.05);hMLH1基因启动子甲基化与hMLH1蛋白表达呈明显的负相关(P<0.05)。结论:hMLH1基因启动子区CpG岛的异常甲基化是引起蛋白表达缺失的主要原因,hMLH1蛋白表达缺失与胃癌的发生有关。 Objective:To investigate the correlation between promoter region 5' - CpG island methylation of hMLH1 and the clinicopathological features of gastric carcinoma and to analyze the correlation between promoter region 5' - CpG island methylation of hMLH1 and expression of hMLH1. Methods:The protein expression and promoter region 5' - CpG island methylation of h MLH1 were detected in 45 cases of surgically resected gastric carci- noma specimens and 21 cases of corresponding normal mucosa tissues using the immunohistoehemistry S - P method and methylation - specific PCR (MSP). Results:The positive rate of promoter region 5' - CpG island methylation of hMLH1 was 33.3% in gastric carcinoma tissues and 0 in adjacent mueosa tissues( P 〈 0.05 ). The positive rate of the expression of hMLH1 protein was 57.8% in gastric carcinoma tissues and 95.2% in adjacent mucosa tissues( P 〈 0.05 ). The protein expression and promoter region 5' - CpG island methylation of hMLH1 have no correlation with clinicopathological param- etem of gastric carcinoma( P 〉 0.05 ). Expression of hMLH1 protein in gastric carcinoma was associated with promoter region 5' - CpG island methylation of hMLH1. Conclusion:Promoter region 5' - CpG island abnormal methylatiou of hMLH1 may be a major reason of the deletion of expression of hbiLH1 protein,deletion of expression of hMLH1 protein correlate with occurrence and development of gastric carcinoma.
