摘要
目的探讨儿童肺炎支原体肺炎的临床特征、影像学特点及治疗方法,以提高早期诊断及治疗水平。方法将该科2011年1月—2012年12月收治的143例肺炎支原体肺炎患儿分成婴幼儿组(0～3岁)、学龄前组(3～6岁)和学龄儿组(6～14岁),回顾性分析、对比3组的临床资料并对其中重症病例的治疗进行初步探讨。结果婴幼儿常伴有喘息、痰咳,影像学以斑片状阴影多见;学龄组、学龄前组各观察指标间无显著性差异,常见症状为高热、刺激性干咳,影像学以节段性、大片状阴影多见。红霉素或阿奇霉素治疗效果良好;重症支原体肺炎病例,加用糖皮质激素结合纤维支气管镜下肺泡灌洗治疗,能收到较好的效果。结论疑似病例应尽早完善相关病原学检查,可早期使用大环内酯类抗生素治疗;重症病例可早期应用糖皮质激素,合并肺不张者,可配合行纤维支气管灌洗。
Objective To explore the clinical features,imaging findings and treatment in children with mycoplasma pneumoniae pneumonia（MPP）,in order to improve the level of early diagnosis and treatment.Methods From January 2011 to December 2012,143 cases of MPP were assigned into infant group（0 ～ 3 years old）,preschool child group（3 ～ 6 years old） and school-age child group（6 ～ 14 years old）.Clinical data of three groups were retrospectively analysed,and the treatment of severe cases among these MPP patients was discussed.Results Infants were often accompanied by wheezing,phlegm cough.The image changes showed patchy shadows mostly;Preschool children had little difference in the observed indicators compared with school-age children.High fever,the irritating dry cough were the common symptoms,and more lobes and segmental lesions had been observed.Therapy with erythromycin or azithromycin should get the effective outcome.Therapy with glucocorticoi,in addition to bronchofiberoscope lavages should be prescribed for these severe cases.Conclusions Suspected cases can be treated with macrolide antibiotics regularly and performed the diagnostic testing if available in a clinically relevant time.Glucocorticoi should be administered in those severe cases at early stage,and those who combined with atelectasis would get benefit from bronchofiberoscope lavages.
