摘要
目的探讨二维超声对妊娠期糖尿病胎儿心脏发育的测量价值。方法选取血糖控制良好的妊娠期糖尿病孕妇胎儿39例,正常妊娠胎儿106例作为对照组。运用二维超声测量胎儿的心脏横径、胸廓横径、左右心房及左右心室横径、主动脉内径、肺动脉内径、主动脉峡部、降主动脉及动脉导管内径。结果血糖控制良好的妊娠期糖尿病孕妇胎儿和正常妊娠胎儿心脏横径、胸廓横径、左右心房及左右心室横径、主动脉内径、肺动脉内径、主动脉峡部、降主动脉及动脉导管内径均随孕周的增加而增长。两组各测量指标相比,差异均无显著性,P>0.05。血糖控制良好的妊娠期糖尿病孕妇胎儿和正常妊娠胎儿肺动脉内径大于主动脉内径,右心房横径大于左心房横径,P<0.05;右心室横径与左心室横径相比差异无显著性,P>0.05。结论妊娠期糖尿病孕妇若血糖控制良好,则胎儿心脏结构无明显变化,胎儿心脏生长发育规律与正常妊娠胎儿一样,随孕周增长逐渐成熟。
Objective: To investigate the practical value of two dimensional ultrasound on evaluation growth of fetal heart in gestational diabetes mellitus pregnancies.Methods: 39 well controlled gestational diabetes mellitus(GDM) pregnancies with 106 normal group were studied.The transverse diameter of heart,thorax,left and right atrium,left and right ventricle,inner diameters of aorta,pulmonary artery,arterial duct,descending aorta,aortic isthmus were measured by two-dimensional ultrasound.Results: The transverse diameter of heart,thorax,left and right atrium,left and right ventricle,inner diameters of aorta,pulmonary artery,arterial duct,descending aorta together with aortic isthmus increased with gestational age in well controlled GDM and normal pregnant fetuses.The measurement indicators between two groups have no difference.The inner diameter of pulmonary artery was larger than that of aorta,the transverse diameter of right atrium was larger than that of left one.There was no difference between transverse diameter of right ventricle and left ventricle.Conclusions: The structure of fetal heart will have no change and fetal cardiac anatomy gradually became mature in GDM as well as normal pregnancies if blood glucose is controlled well.
