摘要
目的探讨大鱼际桡侧逆行岛状瓣修复拇指背侧软组织缺损的临床疗效。方法采用大鱼际桡侧逆行岛状皮瓣修复9例拇指背侧软组织缺损患者,缺损范围:2.0 cm×1.0 cm～3.0 cm×2.5 cm。切取皮瓣面积:2.5 cm×1.5 cm～3.5 cm×3.0 cm。供区直接拉拢缝合或游离植皮修复。结果 9例皮瓣全部成活,创面一期愈合。患者均获随访,时间6～10个月。皮瓣外形满意,质地优良、耐磨,无溃疡发生。术后6个月皮瓣两点辨别觉为8～10 mm。术后6个月手指功能参照中华医学会手外科学会上肢功能评定试用标准评定:优5例,良4例,供区外观及功能无明显影响。结论选用大鱼际桡侧逆行岛状瓣修复拇指背侧软组织缺损,操作简单,功能及外形好,疗效优良。
Objective To explore the clinical effect of greater thenar palmer reversed island flap in repairing of thumb dorsal soft tissue defect. Methods 9 cases of thumb dorsal soft tissue defect were repaired with greater thenar palmer reversed island flap. The range of soft tissue defect was 2. 0cm × 1.0cm - 3.0cm× 2. 5cm. The cut flap ranged from 2. 5cm × 1, 5cm to 3.5cm × 3. 0cm. The donor sites were sutured directly or covered with skin flap. Results All the 9 flaps survived. All cases healed by the first intention. All patients were followed up for 6 - 10 months. All flaps showed good appearance, fine texture, wear-resisting and no ulcer. The two-point discrimination postoperative 6 months ranged for 8 - 10mm. The functions of thumb were assessed according to the trial standard of the function of the upper limb peripheral nerve established by Chinese Medical Surgery of the Hand Association, 5 cases were excellent, and 4 good. Donor site appearance and function had no significant impact. Conclusions The greater thenar palmer reversed island flap in repairing of thumb soft tissue defect has advantages of simple performance, good postoperative function and appearance.
